Looking to RN’s to join our team!
We are a well-established hospice company looking to add to our growing team!! Preferred Hospice has 6 offices in Missouri serving over 65 counties in the state. Those offices are in Dexter, Farmington, St Louis, Columbia, Lexington, and Ozark. Each office serves patients in a 60-mile radius around that office.
We are currently looking for a Full Time RN Case Manager to serve patients in the Lexington, MO area. That office sees patients in all or part of the following counties: Platte, Clay, Ray, Caldwell, Carroll, Jackson, Lafayette, Cass, Johnson, Pettis, Saline.
**SIGN ON BONUS AVAILABLE UNTIL THE END OF March**
The hospice nurse spends most of their workday in their car driving to see patients within a set geographical area. Some required meetings do occur at the office on occasion. This job affords a lot of autonomy as well as great teamwork.
DUTIES:
Hospice RN Case Managers see patients in private homes, assisted living facilities, skilled nursing facilities, and anywhere a patient calls home.
The RN Case Manager performs a head-to-toe assessment, provides symptom management and skilled care to patients facing terminal illnesses as well and providing emotional support and education on death and dying.
The RN Case Manager supervises LPNs and CNAs, monitors medication lists and appropriateness of said mediations, assesses for DME needs, provides wound care and other treatments as needed.
The RN Case Manager works with the Interdisciplinary Team made up of RNs, LPNs, CNAs, Social Workers, Chaplains, Physicians, Nurse Practitioners, Bereavement Staff, Volunteers.
QUALIFICATIONS:
The successful candidate must be currently licensed in the State of Missouri as an RN and live with in a commutable distance to the Lexington office.
We are looking for a nurse who exhibit a thorough knowledge of the holistic approach toward patient care and the nursing process that applies to hospice patients and families.
Home Health, Hospice, Long-Term Care experience preferred but not required.
Willing to train the right candidate. A gentle, loving heart is a must. Must be able to provide love and support to patients and families facing a terminal illness. Must be able to work as a team player and represent our company well in the community.
Computer and smart phone skills for EMR charting, email, apps for mileage computation, etc.
We prefer either professional or personal experience in death and grief.
The offices are open Monday through Friday, 8am to 5pm, with some call required. Reliable transportation and car insurance required.
Health, dental, vision insurance available for FT along with 401K and optional coverage including cancer policies, short-term disability, etc.
Hourly rate plus mileage reimbursement.
