Join Bluegrass Care Navigators for a free grief support workshop and meal on Tuesday, March 3 at The Marksbury Family WayPoint Center, located at the Historic Palmer Pharmacy. This event is open to anyone coping with the loss of a loved one. All ages are welcome. Children must attend with an adult.

Bluegrass Care Navigators has been awarded $97,500 by CHI Saint Joseph Health - Saint Joseph London through the CommonSpirit Health Community Health Improvement Grant to improve access to Bluegrass Transitional Care for high-risk patients in Laurel, Jackson and Whitley counties. Bluegrass Transitional Care will provide up to 181 patients with a 30-day bundled plan of care, which includes hospital bedside enrollment, three in-home nurse visits and unlimited 24/7 phone support.

The expansion of Bluegrass Transitional Care directly advances Saint Joseph London’s and BCN's community health priorities by improving access to care, reducing avoidable hospital readmissions, and enhancing long-term health outcomes for some of the region’s most vulnerable patients.
