This event offers an opportunity to connect with others, share experiences, and learn coping strategies in a supportive environment. 💙

Transitional Care makes the move from the hospital to home easier and safer. When you or a loved one is released from the hospital, we encourage you to see if you qualify for Bluegrass Care Navigators’ Transitional Care program! Our nurses provide expert care, education, and support to help you and prevent a return to the hospital.

At Bluegrass PACE Care keeping you or your loved one living safely in their home is our priority, but who is eligible for PACE Care?

✅Individuals that are aged 55 or older
✅They must live in the service area of Fayette, Franklin, Anderson, Woodford or Jessamine counties
✅They need a specific level of care that may otherwise require a nursing home
✅They must be able to live safely in the community with help from PACE
