Bluegrass Clinic Liberty, PLLC, will permanently close its doors as of February 16, 2024.

We've valued the opportunity to serve this community for many years, and it is our wish and objective to ensure a smooth transition for your medical care. We will be happy to continue to provide medical care for you in one of our other offices.

If you would like to transfer care, you can schedule appointments at Stanford at Bates, Miller, and Sims (606) 365-1547 or Bluegrass Clinic, Stanford (606) 365-8338 or in Waynesburg at Waynesburg Clinic (606) 379-6646.

Otherwise, our staff will be available to ensure refills are sent, and record requests are processed. Refills will only be provided for a 30-day supply and only for the next 60 days. Any refills after 60 days must be with your new provider or by seeing us in our Waynesburg or Stanford offices.

We appreciate you allowing our providers to meet your medical needs and for being our loyal customers.

Meet Mindy Allen, PA-C, who recently joined our team after completing the Physician Assistant Program at the University of Kentucky in 2019. She's been working in emergency medicine since graduation.

She loves traveling and spending time with her family, whether it’s on the farm, traveling, or just at home.

Mindy is looking forward to serving the Liberty community and is currently taking walk-ins only every Wednesday & Thursday.
