Libertyville Manor Extended Care Community is a 174 bed skilled nursing and rehabilitation center that offers five levels of specialized care: sub-acute, skilled, intermediate, light intermediate and assisted living. For all levels, our dedicated professional staff is available around the clock to monitor each resident's healthcare needs. This unique continuum of care offers both the resident and the family peace of mind. As a resident's needs change Libertyville Manor's level of care changes to meet those needs, maximizing individual potential and comfort. Libertyville Manor has also developed a sub-acute healthcare program that involves the coordinated services of an interdisciplinary team dedicated to the care of the medically complex individual. This care is made necessary today as a transition from the hospital to home or an alternative to high-acuity medical care.
