With MyChart, you can revisit your provider’s notes anytime to review what was discussed, the plan moving forward, and any recommended treatments. When you have questions, there’s two-way communication between you and your provider without having to call the clinic.

Our resident doctors have completed their medical school and are here for three years to refine their skills in family medicine. Due to their teaching requirements, they can't always see the same patients.

We created "Stepping Up To The Plate," a story that highlights how physicians work together as a team and the trust they place in one another to provide exceptional care.

During the transition, you may not have immediate access to the new patient portal, MyChart. We appreciate your patience and understanding.

Effective March 31, you will no longer have access to any personal health information on our current patient portal, Athena.

Through an integrated care approach, our patients have access to behavioral health specialists at our clinic. Therapists at Stepping Stones work collaboratively with our family medicine providers, ensuring whole-person care for our patients.

If you would like to speak with a therapist, your provider can arrange for a meeting with one.

When we transition to Epic, all of our patient information will no longer be accessible from Athena, our current electronic health record. Patients will need to update their medical history and intake forms. This will ensure that we have all of your current health information in the new system.

You will receive communication from Lincoln Medical Education Partnership with instructions on updating your information.
