Bringing dialysis care directly into the skilled nursing setting allows us to meet patients where they are, reducing disruption, enhancing continuity of care, and supporting better overall experiences for both patients and facility partners.

This opening is a true team effort, and thank you to everyone who helped bring this unit to life.

Looking forward to the impact this team will make in the Montgomeryville community and beyond.

We had a strong turnout, and the engagement and collaboration were great to see.

Thank you to everyone who participated and helped make the day a success!

Today, on , we’re recognizing and thanking our incredible social work teammates for the compassion, advocacy, and support they bring to our dialysis patients every day.

The dialysis journey extends far beyond treatment, and our social workers play a vital role in helping patients navigate the emotional, social, and practical challenges that come with care.

To celebrate, we’re spotlighting two tenured social workers whose impact continues to be felt across Concerto.

Hannah Gallagher, MSW, LSW, who has been with Concerto for four years, shared:
"One part of my work that I've found most rewarding is being able to share human connection with patients who may not often experience that. So many nursing home residents feel isolated, and it's a pleasure to have a conversation with them and show them they are seen and valued."

Syreeta Chism, who joined Concerto in 2022, shared:
"Working at Concerto as a Social Work preceptor provides cyclical rewards. Sharing knowledge with trainees gives me renewed appreciation for what we do. In the clinic setting, I have great working relationships with my colleagues and SNF partners, which makes patient care easier to manage and leads to greater patient satisfaction. It's a win-win!"

Thank you to Hannah, Syreeta, and all of our social work teammates for the meaningful difference you make each day.

As part of our celebration of National Registered Dietitian Nutritionist Day, we’re continuing to highlight some of our tenured dietitians and recognize the lasting impact they make across Concerto. Today we’re spotlighting Katherine Marusarz, Lead Renal Dietitian in Illinois, who has been with Concerto for seven years. 💙

Katherine shared that one of the most meaningful parts of her role is providing nutrition education to patients—helping them navigate a highly restrictive diet and manage the many medications involved in their care. She also values being a resource to fellow dietitians as they work through complex clinical challenges each day.

What has kept Katherine at Concerto for seven years? She credits the strong work-life balance the role provides, along with the opportunity to work side by side with her wonderful dietitian and social work team.

Thank you, Katherine, for the expertise, support, and care you bring to both patients and teammates every day. 👏✨

Today, on Registered Dietitian Nutritionist Day, we’re spotlighting Nicole Sinkler, Renal Dietitian in Maryland, who has been with Concerto for seven years! 💙

Nicole shared that one of the most meaningful parts of her role is the opportunity to educate patients each month and build lasting relationships with them. “Knowing I can make a difference in someone’s life is priceless and very gratifying,” she says. Her goal as a renal dietitian is to help improve lives through nutrition and medication management.

A moment that makes Nicole especially proud is when patients look forward to seeing her each month — a reflection of the trust she has built and their willingness to make changes that support better long-term health. 👏✨

Today, we’re proud to spotlight Tashara Garrett, Renal Dietitian based in Michigan, who is celebrating five years with Concerto Renal Services!

When asked what she finds most rewarding about her work, Tashara shared:

“I find working with our patient population very rewarding. I like working with patients and facility dietitians to make sure patients are not only getting the therapeutic diet they need, but that it also includes some of their favorite foods. I also like to try to bring a smile to their faces.”

We also asked what has kept her at Concerto for five years:

“What’s kept me at Concerto for 5 years is the feeling of being appreciated. It’s hard to find a large company that does right by its employees, and Concerto has done just that. Also having great co-workers and leadership in the nutrition department.”

Thank you, Tashara, for the compassion, expertise, and heart you bring to patient care each day. Your impact is felt by both patients and teammates alike 💚👏
