To best serve our patient's needs, please review the following:

• Ladies must be at least 35 years of age
• Have not had a screening mammogram in the past 12 months
• Have no new breast symptoms (ex: lump/pain/rash/nipple discharge, etc)
• Is not pregnant or breastfeeding
• Must be able to climb/descend steps on mobile without assistance

Information needed to schedule:

• Legal name• Date of birth
• Current address
• Daytime phone number
• Name of physician•
Facility information & date from last mammogram (if applicable)

Patients MUST bring their Photo ID & Insurance Card(s)

Options available for individuals without insurance
• Cash price $161.70
