Medical Assistant (MA) – Care Coordination
(Not Remote–Working with Virtual CareTeam in Longview, TX)

Join Our Team! PhyNet is seeking an MA to work with the care team from a centralized office setting, focusing on remote patient support and care coordination rather than in-person clinical tasks. The role helps ensure ongoing engagement for patients with chronic conditions.

CORE ROLES:
Perform follow-up calls for routine check-ins, medication adherence, and care plan compliance.
Coordinate appointments, referrals, and diagnostic services.
Document interactions and care updates in the EHR.
Assist with tracking preventive care and managing care gap lists.
Communicate with patients and providers to support continuity of care.

CERTIFICATION / EXPERIENCE:
Certified MA preferred; BLS
Prior experience in remote patient coordination or outpatient care setting is a plus.

Keep in mind: Caller IDs and email addresses can be faked, so if you're not sure who's calling, call us at 1-800-MEDICARE (1-800-633-4227). And remember, Medicare will never contact you unless you've contacted us first.

It's important to know the signs of a Medicare scam so you can protect yourself. To get your personal information, like your Medicare number, scammers may call, text and email saying things like:

"You need to activate or renew your Medicare card."

"We'd like to send you a free genetic testing kit."

"Medicare has authorized us to send you a free (knee, back or elbow) brace."

"You qualify for a refund on your Medicare costs."

Remember, never give your Medicare card or number to anyone except your doctor or people you know should have it.

PhyNet Lindale is pleased to welcome Dr Ellen Remenchik. Dr Remenchik is available for scheduled appointments from Mon. to Fri. from 8am - 5pm and walk-ins from 8am - 4pm. For appointments, call the PhyNet Lindale Medical Clinic at 903-882-3194.

Dr Remenchik brings over 30 years of diverse health
care experience to PhyNet Lindale. With a focus on
Internal Medicine and Occupational/Preventive Medicine, she has a strong background in academic and clinical
medicine, as well as research, serving in various roles at institutions including Baylor College of Medicine, UT Tyler Health Science Center, Mayo College of Medicine, and UTHSCSA.

She earned her undergraduate degree Cum Laude from the University of Illinois at Chicago Circle, her MD from Texas A&M, and her MPH from the UT School of Public Health in Houston.

Her more recent work has included geriatric care, rehabilitation, and medical education. At UTHSCSA, she taught medical students and served on the Medical School Student Selection Committee. She has also held Medical Director positions in both academic and private practice settings.Ellen has received several honors for her contributions to teaching and medicine, including UT Tyler’s Occupational Medicine Preceptor of the Year.

She was also recognized by NASA with the Lyndon B. Johnson Space Center Group Achievement Award for her work in the 1994 Astronaut Candidate Medical Selection, as well as a Certificate of Appreciation for her support of the JSC Total Health Program. She is currently board-certified in Internal Medicine and Occupational Medicine from NBPAS.

In her free time, Ellen enjoys all kinds of music and hiking in the great outdoors.
