CLINIC HAPPENINGS
Doctor Christopher Mocek, MD was invited to speak to all of the CARTI CANCER CENTER physicians to provide an update on proper prescribing of opiods for 2025. He covered the CDC’s updated twelve opiod prescribing guidelines for physicians & various other related government regulations for safety & compliance (state & federal) in the proper use of these medications for acute & chronic pain relief. He also discussed new alternative medications to opiods for pain relief!

REAL MEDICAL BREAKTHROUGHS IN CHRONIC PAIN STUDIES
A new non-opioid molecule that acts like a long-lasting local “anesthetic” for chronic pain.
Researchers at University at Buffalo (UB) developed a peptide-based molecule that when locally injected in animal models relieved chronic pain behaviour for up to three weeks! They targeted the interaction between the scaffolding protein Magi-1 and the pain-transmitting ion channel NaV1.8 (a sodium channel known in pain signalling). 
Significance: This is very promising because it: (a) is non-opioid, (b) provides long-duration pain relief, (c) is selective (pain-fibers) rather than blocking all sensation.
Caveat: Again pre-clinical (in animals). Local injection may limit use (vs systemic treatment). Not yet in humans.
This new peptide holds out hope for those suffering with chronic pain where the traditional pain relief medications are not working!

A SAFER ALTERNATIVE MEDICATION COMBINATION FOR CHRONIC PAIN RELIEF
Some of our patients who have to take opiods daily for advanced spine conditions causing chronic pain or our patients with mild to moderate cancer related pain have been able to transition to the buprenorphine pain patch (one patch per week) & oral tramadol for breakthrough pain. This combination for chronic pain is clearly safer than other traditional opioids with much less chance of the medications causing over sedation or slowing a person’s breathing rate down. Another added benefit is that this combination of medication has the option for refills so you don’t have to come for a visit every month to get the medication once you are on a stable & effective dosage. In clinical studies these two drugs did not suppress the immune system while your body is trying to fight the cancer. (Both drugs are now available in the generic form to reduce costs.)

A NEW VERY PROMISING POTENTIAL FUTURE LUMBAR DISC PROCEDURE FOR PAIN RELIEF USING OZONE!
A minimally invasive spine procedure injecting a controlled oxygen–ozone mix into a lumbar disc to relieve pain & inflammation from contained disc herniation.
• Goal:
Shrink the herniated disc, reduce nerve pressure, & improve back or leg pain without open surgery.
• Procedure:
Outpatient, image-guided needle injection — typically 20–30 minutes, minimal recovery time.
• FDA status:
Granted Breakthrough Device Designation (Aug 2025) for treating lumbar disc herniation.
(Not yet FDA-cleared or commercially available in the U.S.)
• Results (from ozone studies):
• Pain/function improvement in ≈ 70–80% of cases
• Non-inferior to microdiscectomy at 6 months (select patients)
• Durable benefit seen in ~75% at 5–10 years
• Safety:
Low reported complication rate when done under imaging guidance.
• Best candidates:
Adults with contained lumbar disc herniation, persistent pain despite conservative care, and no major instability or nerve damage.
• Status:
Promising but investigational — clinical trials ongoing to confirm long-term safety and efficacy.
