Our amazing therapy team at Courtyard Health and Rehab!!

Mr. Modi came to Courtyard Health and Rehabilitation needing skilled rehab after experiencing weakness and an infection that made it difficult for him to care for himself. With the support of our dedicated therapy team, he worked hard on strengthening, balance, endurance, and daily living skills.

Today, Mr. Modi is able to return home with his sister and get back to doing the things he was able to do before. When asked about his experience, he shared, “They were all very supportive and very nice. They got me back to doing what I was able to do before and I am so happy!”

🌟Team Spotlight: Anna Furlong, COTA and Director Of Rehab at Lake Village Rehabilitation and Care Center🌟

We are excited to spotlight Anna on her promotion to the DOR position! Her heart, positive energy, and servant leadership make a powerful impact on our therapy team in Lake Village, and we are so proud to have her in this role.

Anna earned her Associate of Applied Science in Occupational Therapy from the University of Louisiana at Monroe, after completing her Bachelor of Science in Health Studies at Arkansas State University. She began practicing as a COTA in April 2024, gaining experience in both home health and skilled nursing facilities, and joined the ProCare family in December 2024.

Outside of work, Anna married her high school sweetheart in December 2023 and is joyfully preparing to welcome a precious baby boy this July. She loves spending time with family, traveling (especially beach trips 🌊), baking, listening to music, and crafting.

What Anna loves most about being a COTA is helping patients regain independence through improving strength and range of motion, balance, coordination, cognition, safety awareness, and functional skills. She is passionate about implementing adaptive equipment and modifying daily tasks to support success with activities of daily living.

Anna said, “The most rewarding part is seeing the confidence and joy patients gain as they accomplish their goals.”

Her favorite OT quote says it best:

Let us introduce you to Mary Ferguson, one of our incredible Occupational Therapists at The Green House Cottages of Homewood in Mena, AR.

Mary brings 25 years of experience to her field and a heart fully devoted to caregiving. She is a loving wife, a proud mother of four, and a grandmother to many—roles that deeply influence the compassion she brings to her work every day.

In addition to her Occupational Therapy degree, Mary is LSVT–certified and a geriatric specialist. She is the creator of a Parkinson’s program in her community, serving both patients and caregivers with education, movement, and support. Mary also generously donates her time and resources to uplift and strengthen her community.

Among her many accomplishments, Mary dreams of becoming part of a hippotherapy program in her area and is passionate about educating individuals with dementia and supporting their families through every stage of the journey.

Mary is the definition of a true caregiver—selfless, dedicated, and always giving back to those in need. We are honored to have her as part of our team.

I am a physical therapist assistant, I graduated from SouthArk College in El Dorado, AR in May 2025 with an Associate of Applied Science degree in Physical Therapist Assistant! My name is Tyler Furlong. I am a Physical Therapist Assistant, dad-to-be, and an avid sports fan who loves traveling, video games, and chess. Go Bucs! I have nearly five years of experience providing care in both SNF and home health environments.
