When you become a patient at MRP after attending physical therapy elsewhere, you can tell it’s different. The vibe is different, our approach is different, and our mindset is different.

As physical therapists with Michigan Rehab & Performance, we demand excellence of ourselves because that is how we best serve our patients and our community. If your PT is providing anything other than the best quality of care possible, you should look elsewhere.

The healthcare landscape is changing, and in a low of ways it is not for the better.

Costs keep going up, reimbursement keeps going down, and the amount of good that your insurance company actually does for you seems to get smaller and smaller.

One good thing to come out of this, though, is the cash-based physical therapy movement. Still new to many, cash-based PT as a whole allows individuals to have more say in where they go for their care but also what quality they receive.

By deciding to pay out of pocket, many individuals can receive a significantly higher level of care (compared to a lot of these big PT companies) for a similar, if not CHEAPER, price tag. Many people will actually save money by going out of network because the price of 1-2 visits may be less than paying a high copay for several visits per week over the course of a month.

Cash-based PTs have made a choice to avoid the insurance hassles, offer affordable care, and deliver a personalized experience to get you back to (or keep you feeling) your best.

“5 stars, 10 stars, 50 stars, I give whatever highest rating I can give Dr Ryan. So kind and knowledgeable, he quickly isolated and treated the cause of my pain that I initially came to him for. He’s a crossfit and fitness fan too, so he’s tuned right in to the athletic routines. Very observant to the whole patient too, he also picked up on other mobility and strength issues that I was battling. When it comes to injuries among runners, the knee accounts for close to half of all injuries.

16% of knee injuries are considered patellofemoral pain syndrome, which is a fancy way of say the tissue around your knee cap is irritated.

8% are generated from the IT Band, which can be felt as pain on the outside of the thigh or outer part of the knee.

Other injuries of the knee in runners include patellar tendinopathy, meniscal issues or osteoarthritis that is aggravated.

The great news about all of these conditions are that they do very well with a structured rehab plan and a period of gait re-training.

TL;DR: Cortisone injections may provide short term relief and functional improvements with rotator cuff tendinopathy, but at mid- and long-term follow ups show no additional benefits and may have potential for adverse effects.

On this research Friday we’re going to dig into this study by Lazzarini et al in the Physical Therapy & Rehabilitation Journal. In this systematic review and meta analysis, this team looked to compare the effectiveness of subacromial corticosteroid injections (or cortisone as you may know it) in combination with anesthetic injection or any other PT interventions to just physical therapy interventions alone in patients with rotator cuff tendinopathy. This is a pretty big study. In total, they looked at 15 randomized controlled trials which added up to over 1700 participants included. In terms of outcomes, they looked at pain, level of function, quality of life and patient-rated improvement. As far as results, they found that at short term, the corticosteroid injections coupled with PT may result in small to moderate pain and functional improvements compared to PT alone. This isn’t that surprising and lines up with what we already know. Cortisone is a powerful anti inflammatory, and when you quickly drop their inflammation they feel better quickly. Conversely, corticosteroid injection alone compared to PT did not seem more effective at improving pain and function in most of the studies. Furthermore, at mid and long term follow ups, corticosteroid injections did not seem more effective than any other PT interventions. We love studies like this because it highlights the power of physical therapy, but also gives us more information to help our patients make informed decisions about injections. There may be a time and place where injections are recommended or even necessary, but they are often a great example of a situation where the short term gain is not worth the long term cost, as more studies come out that highlight the potential negative effects on tissue health as well as the lack of long term benefits.

EXERCISE HELPS REDUCE PAIN!!!

We hear it all the time - I stopped going to the gym when I hurt my (insert body part here).

That’s the last thing you should do!

Do we sometimes have to modify/alter training due to pain or injury. Of course, but we should NEVER stop training.

In this research study, the group looked to find out if doing a quadriceps exercise at variable intensities would have a local and/or global effect on pain pressure threshold.

What they found was that both had global effects on pain pressure threshold meaning that any exercise (at the right intensity) will cause systemic change in pain levels. Furthermore, high intensity exercise to the local tissue has an immediate effect on reducing pain sensitivity.

Bottom line is pain does not mean you have to stop training. Modify or alter as needed, and worst case scenario we can work another area while addressing the local tissue.
