A solid overhead position is essential in CrossFit, for everything from kipping on the pull up bar to snatches and hand stand push ups.

One common limiting factor we see is lat length. If your lats are tight, it will prevent you from getting into a good position without compensating somewhere else.

Give this test a try and see if your lats could be limiting your potential!

Every October we celebrate .

Join us in welcoming our newest PT intern, Moe! Moe is a second year student at Wayne State () and joining us for his current rotation.

A little bit about Moe:
- Moe helps out in the anatomy lab preparing/dissecting the cadavers to be used for class
- Outside of class, he helps run a food truck and has an online personal training business
- He loves cooking and finding ways to turn cheat meals into high protein, healthier options
- His favorite thing to do is get a good lift in
- Favorite energy drink: C4 Watermelon Jolly Rancher
- He is one of five brothers!
- One of his brothers is a chiropractor
- His ideal patient to work with is the injured lifter

We’re going to be back alongside this weekend for Motor City Monsters 2025!

Really looking forward to enjoying the nice fall weather and watching some crazy strong humans do what they do best. We will be on-site providing any prep or recovery services these athletes need (or any spectators!).

This patient is a 17 year old male lacrosse player who had ACL reconstruction with quad tendon graft back in May 2025. He came to us at 14 weeks post-op after attending physical therapy at a different facility and not getting the attention, intensity, or results he needed and deserved.

Like with a lot of ACL patients, in these early weeks our focus has been primarily beefing up his quad, but also addressing bilateral hamstring deficits for his long term knee health.

He’s only been coming in twice per week, and he’s been religious with his home exercise program. We’ve used minimal manual therapy. His exercise list isn’t 15 things long and he hasn’t had to come in 3x/week since surgery to get these results.

Just strategic, targeted, and dialed-in loading strategies and programming.

I had a discussion with a patient the other day that made me want to revisit some set-up tips.

Set-up is always important, but with the deadlift especially it can make or break your lift.

These are by no means the only faults I see but some of the more common.

Fault #1: Too Vertical
If you get “squatty” with your deadlift, you make it hard to use your posterior chain as well as create a forward bar path making it tougher to move more weight.

Fault #2: Too Horizontal
When you start too flat, it is near impossible to get any leg drive which places the torque squarely on your lower back.

Fault #3: Poor Lat Engagement
Lat engagement is huge for keeping the bar within your center of mass and leveraging physics to your advantage. When the bar drifts away from you, it makes it harder on your lower back and limits your ability to lift heavier weight.

“5 stars, 10 stars, 50 stars, I give whatever highest rating I can give Dr Ryan. So kind and knowledgeable, he quickly isolated and treated the cause of my pain that I initially came to him for. He’s a crossfit and fitness fan too, so he’s tuned right in to the athletic routines. Very observant to the whole patient too, he also picked up on other mobility and strength issues that I was battling. I feel so much better and am armed with detailed exercises to keep up with at home. Many thanks Dr Ryan! Def recommend!!”

When it comes to injuries among runners, the knee accounts for close to half of all injuries.

16% of knee injuries are considered patellofemoral pain syndrome, which is a fancy way of say the tissue around your knee cap is irritated.

8% are generated from the IT Band, which can be felt as pain on the outside of the thigh or outer part of the knee.

Other injuries of the knee in runners include patellar tendinopathy, meniscal issues or osteoarthritis that is aggravated.

The great news about all of these conditions are that they do very well with a structured rehab plan and a period of gait re-training.

TL;DR: Cortisone injections may provide short term relief and functional improvements with rotator cuff tendinopathy, but at mid- and long-term follow ups show no additional benefits and may have potential for adverse effects.

On this research Friday we’re going to dig into this study by Lazzarini et al in the Physical Therapy & Rehabilitation Journal. In this systematic review and meta analysis, this team looked to compare the effectiveness of subacromial corticosteroid injections (or cortisone as you may know it) in combination with anesthetic injection or any other PT interventions to just physical therapy interventions alone in patients with rotator cuff tendinopathy. This is a pretty big study. In total, they looked at 15 randomized controlled trials which added up to over 1700 participants included. In terms of outcomes, they looked at pain, level of function, quality of life and patient-rated improvement. As far as results, they found that at short term, the corticosteroid injections coupled with PT may result in small to moderate pain and functional improvements compared to PT alone. This isn’t that surprising and lines up with what we already know. Cortisone is a powerful anti inflammatory, and when you quickly drop their inflammation they feel better quickly. Conversely, corticosteroid injection alone compared to PT did not seem more effective at improving pain and function in most of the studies. Furthermore, at mid and long term follow ups, corticosteroid injections did not seem more effective than any other PT interventions. We love studies like this because it highlights the power of physical therapy, but also gives us more information to help our patients make informed decisions about injections. There may be a time and place where injections are recommended or even necessary, but they are often a great example of a situation where the short term gain is not worth the long term cost, as more studies come out that highlight the potential negative effects on tissue health as well as the lack of long term benefits.
