If you or a loved one is getting ready for Medicare, you may hear about CMS Form L564 — but what does it actually do?
👉 CMS-L564 = Request for Employment Information
This form is used to prove that you had creditable health coverage through an employer (yours or your spouse’s). It’s especially important if you’re signing up for Medicare Part B after turning 65.
