Central Mountain Physical Therapy & Fitness Center Central Mountain Physical Therapy & Fitness Center is a private practice in Lock Haven, PA located between State College and Williamsport.

We not only offer physical therapy but also have a fitness center that is open to the public. We accept most insurances for PT. Our fitness rates are as follows: $40 Single Membership, $55 Spousal Membership. We also have yearly rates: $440 Single & $605 Spousal, which saves you on one full month!

ATTENTION HIGHMARK, SILVER SNEAKERS MEMBERS:
You should have received notice from Highmark that you will no longer have Silver Sneakers coverage in the new year. Highmark will be switching to the FitOn Health Fitness program as of January 1, 2025.

DO NOT WORRY!
We will help you sign up for the new FitOn Health program in addition to requesting our facility being nominated as a site for your continued Fitness Membership....

At your first visit in the new year, please let us know you have Highmark insurance as your secondary to Medicare. Only "Highmark" Silver Sneaker members will have to go through this sign-up process, but we will direct you on what to do during your first visit in the New Year!

Please Note: The FitOn Health program only allows you to select one location for your health fitness plan. If you are currently visiting more than one site through Silver Sneakers, you will have to choose one site as your primary place to visit.
