Possible complications from cosmetic surgery

I was asked by some of my patients to discuss the potential complications from breast and tummy cosmetic surgery. The reason I chose my area of surgical focus is because benefits for patients are high, and the risks are low. Most patients are healthy and at an ideal body weight, setting them up for a very low complication rate. Regardless, complications can arise, and I spend a lot of time during my consult discussing these. Ultimately, in order for me to offer a patient surgery, I have to feel strongly that I can deliver a significant, long-lasting improvement, but also that the benefits far outweigh the risks.

Lower Eyelid Surgery 👁️👁️

This patient was bothered by his lower eyelid “bags” and “tired” appearance. He desired to look more refreshed, while remaining natural-looking and masculine. He underwent a lower blepharoplasty with fat grafting to his cheek and lower eyelid. This provided a more blended/youthful appearance to her lid-cheek junction.

Lower blepharoplasty is an outpatient surgery. Patients should expect 2-3 wks of bruising and swelling, with a final result between 3-6 months. This patient is 6 months out from surgery.

360 degree Tummy Tuck

After pregnancies and > 100 lbs of weight loss this patient was left with excess skin that wrapped circumferentially around her entire trunk. A standard tummy tuck (incision from anterior hip-to-hip) would NOT have been a good option for her - it would’ve left her with redundant skin on her lateral hips and back, and a resultant suboptimal shape/contour. She instead underwent a 360 degree tummy tuck aka Belt lipectomy aka Lower body lift.

Her incision goes circumferentially around her waist, but is all concealed beneath the underwear/bikini line. Weight-loss patients often also have excess skin in their upper p***c area, so I commonly perform a “mons lift” in conjunction, to provide a more youthful appearance to this area.

This patient is 1 year out from surgery and describes how much better she fits into dresses and bathing suits, as well as how much more active she’s is because the excess skin no longer restricts her.

This surgery is performed as an outpatient. The recovery is approximately 4-6 weeks long, with 2 weeks off of work.

Upper Eyelid Surgery - 1 yr Result

This younger patient developed excess skin in her upper eyelids that bothered her as it gave her a “tired” appearance. These changes are usually due to aging, but in this situation, it was more genetic given her younger age. Patients will oftentimes mention that their parents had similar eyelids at a younger age. She underwent an upper blepharoplasty, which is a 1-hr outpatient surgery with 3-4 wk recovery.

Her incision is very well-concealed, a thin line within the natural creases of her upper eyelid

This patient is 12 months out from surgery. As you can see, her breast lift “lollipop” incisions have a light pink hue will continue to fade up until the 2 year mark. “Mommy Makeover” surgeries are performed as an outpatient, with a 4 wk recovery, 2 wks off work, and 4-6 months until final result.

This or That: Breast Implant Edition

Regarding implant placement above or below the muscle (aka subfascial/subglandular vs. dual-plane/submuscular), I perform both. It depends on the patients preferences and anatomy. With the newer implants, because of its smooth-silk surface, they illicit a lower inflammatory response and therefore less risk of capsular contracture. As long as the patient has a sufficient amount of breast tissue, implants can be placed above the muscle and still look and feel very natural. However, some thinner or post-partum patients with less breast tissue may be better candidates for under the muscle, because it adds an extra layer of tissue to camouflage the implant it, allowing the breast implant to look more natural. During your consultation, we will talk extensively about the pros and cons, and which is the best fit for your body
