Advanced lower jaw necrosis is rare, but very serious. It occurs when the jawbone fails to heal and begins to die, often associated with certain chemotherapy or antiresorptive medications.

This patient, in their early 90s, presented with sepsis stemming from advanced mandibular necrosis. Imaging showed extensive bony involvement requiring surgical resection. Given their age and overall health, reconstruction was not pursued. The priority instead was eliminating the infection and restoring comfort and function to improve quality of life.
