We provide 24-hour skilled care and rehabilitation services in a comfortable and friendly environment. Our team of experienced professionals is here to assist you or your family member with a variety of services during there stay here. Our Medical Director coordinates and, where possible, improves upon medical services. Our Director of Nursing oversees all aspects of health care within the facility. Our Physiatrist oversees the Rehabilitation services offered here at Courtyard Care Center.nursing

Wound care
Complex post surgical needs
Trach Care
Respite stays
Enteral feeding program
Catheter care
Colostomy Care
Off-site dialysis
Illiostomy Care
Nutrition & hydration programs
Medication management & education
Diabetic management & education
Restorative nursing
Pain management
Hospice program


Restorative Nursing Program

Therapy and nursing staff develop individualized treatment plans for residents in the areas of:

Ambulation
Range of motion
Activities of daily living
Positioning and alignment
Skin integrity and wound management
Contracture management including whirlpool treatments
Bowel and bladder training
Rehabilitative dining
Strengthening and restraint reduction


Dietary Program

Examples of our dietary services:

Provide meals to meet specific dietary and therapeutic needs, according to physician orders. (Please inform the dietary supervisor in advance regarding all substitute menu requests.) Special Select Menu allows for greater resident choice of meals
Oral supplemental diets are available
Regular nutritional assessments from our registered dietitian
Alternate Menu includes but not limited to:
Fruit Plate with Cottage Cheese
Cobb Salad
Hamburgers and Hot Dogs
Burritos
Tuna and Egg Salad or Sandwiches
Baked Potatoes
Pizza

Meet our Courtyard Doctors 🌳 Dr. Thomas Kim, DO.
Dr. Kim is a Board Certified Physical Medicine and Rehabilitation (PM&R) physician. He received his Bachelors from the University of Florida and his Masters’ from the University of South Florida in Molecular Medicine. Obtained medical degree from Lake Erie College of Osteopathic Medicine. Finished PM&R speciality residency at ACGME accredited Casa Colina Hospital/Western University. Dr Kim is now providing a full spectrum of medical care from outpatient musculoskeletal, sports, pain medicine, neuro-rehabilitation and inpatient rehabilitation.

Meet Our Courtyard Doctors 🌳 Dr. Kathy Palatnik, MD, CWSP.
Dr. Palatnik was born in Hollywood, Florida soon moving to Hollywood, California at six years of age. Growing up in Los Angeles gave her many opportunities that eventually led to her exposure to and love of medicine. After shadowing her stepfather on nephrology rounds and with the support of her family, she attended UCLA and graduated with a major in Physiological Science. Upon graduation, she worked at the UCLA Department of Cardiothoracic Surgery, where she was able to participate in multiple research projects, in addition to heading the UCLA Heart and Lung Transplant Organ Procurement Team, a team in place to assist in and coordinate the transport of donated hearts and lungs across the country. She soon attended the David Geffen School of Medicine at UCLA to obtain her medical doctorate. After that time, she attended residency in General Surgery at Harbor-UCLA Medical Center in Torrance, CA. Since then, she has been providing her surgical/wound care expertise with Advantage Surgical and Wound Care.
Her hobbies include travel, film, classic automobiles and music.

Meet Our Courtyard Doctors 🌳 Dr. Nga, D.O. was born in Battambang, Cambodia. He and his family fled Cambodia to live in a refugee camp in Thailand from 1984-1988. They settled in Stockton, CA after arriving to the United States. He received his B.S. in Chemical Engineering- Biomedical Option at the University of California, Los Angeles. He went on to obtain his Doctor of Osteopathy Medicine (D.O.) from Touro University- Nevada. Dr. Nga completed his Internal Medicine training at St. Mary’s Medical Center, Long Beach, where he was chief resident. Currently Dr. Nga, works both in his primary care clinic and as a hospitalist medicine in the hospital. He enjoys doing both because it allow him to provide continuity of care for patients in both clinic and the hospital. Dr. Nga believes that to achieve healthy life style, we must work on achieving a balance between the body, mind and soul.
