We offer a wide range of individualized care and services. We are a Continuing Care Retirement Community.

The Peaks Care Center offers long term care, short term rehabilitation, assisted living, and independent living. We offer Independent Living, Assisted Living, Long Term Care, and short term rehab at our Frontier Therapy Center. We also offer Physical, Occupational, and Speech Therapy to work with those that would benefit from rehab. We welcome you to contact our Admissions Department if you have any questions and would like to come tour our building!

Janet’s decades of dedication, compassion, and excellence in nursing have touched countless lives here in Longmont. To be chosen as the statewide winner is a tremendous honor, and we couldn’t be prouder.

Breakfast in the Courtyard! 🧇🥓☀️

This morning our assisted living residents enjoyed a beautiful breakfast outdoors, complete with fluffy waffles, crispy bacon, savory sausage, and refreshing drinks. It was the perfect way to start the day with great food, fresh air, and wonderful company! 💛

🌺🌴 Aloha from Our Annual Hawaiian Luau! 🌴🌺

Yesterday, our community came together to celebrate our much-loved Hawaiian Luau, and what a wonderful time it was! 💃🌺 With delicious food, tropical drinks, live festive music, and beautiful Hawaiian dancers, the day was filled with joy and the true spirit of aloha.

A big mahalo to everyone who joined in making the event so special – from our residents and families to our amazing team who made it all possible. 🌞🍍🌊

🐾🐶 Bring Your Dog to Work Day! 🐶🐾

Today was extra special as our team celebrated Bring Your Dog to Work Day! Our workplace was filled with wagging tails, wet noses, and lots of joy. 🐕💙

Not only did our furry friends brighten everyone’s day, but they also reminded us of the comfort, companionship, and love that pets bring into our lives.

A big thank you to our staff who joined in and shared their pups with us—we loved every minute! 🐾✨
