Ever wonder what happens if you lose weight after DIEP flap?

My amazing patient had a DIEP flap years ago and came back after a signficant GLP1 related weight loss for a quick revision to tighten up her breast skin-- just as after one loses weight with a natural breast, a DIEP can get deflated with greater than 15 lb weight loss . We refined her result and now she feels great. ✨
It’s such a privilege to be part of these ongoing chapters, creating results that grow and change right alongside each patient. 💕

Breast reconstruction is often a multi-stage process, typically completed over 2–3 surgeries. This patient came to Dr. Slack for a revision after her initial reconstruction with tissue expanders performed at an outside practice. We exchanged the expanders for DIEP flaps, resulting in a softer, more natural appearance with smooth contours and no implants. Another benefit was the improved abdominal contour as you can see on the second lateral view.

Revision surgery is an option—for comfort, aesthetics, or both. Every step is about restoring confidence and feeling whole again.

Breast Augmentation with Fat Grafting, Before ➡️ After | She chose saline implants in a dual plane position with fat grafting. Why fat grafting? While implants provide volume and projection, fat grafting helps smooth transitions, enhance cleavage, and add subtle fullness to the upper pole—especially in thinner patients or those with asymmetries. It's a powerful tool for creating softer, more natural-looking contours.

This transformation is about so much more than what meets the eye.

This remarkable woman faced a breast cancer diagnosis while undergoing a major weight loss journey—two life-altering challenges that she approached with grit, grace, and a sense of humor that brightened every room she walked into.

Her before and after photos show a striking physical change, but they don’t capture the full story of her resilience, her kindness, or the countless moments of laughter we shared along the way.

Because of the complexity of her case—addressing changes from both cancer treatment and massive weight loss—her reconstruction was staged over time. These types of operations require thoughtful sequencing to achieve the best functional and aesthetic outcomes. But they also allow something else to develop: trust. Over time, the surgical plan evolves, and so does the relationship between patient and surgeon.
