A s***matocele is a common but often overlooked cause of testicular pain.
It is a cyst that forms on the epididymis, the structure attached to the testicle where s***m matures. Over time the cyst can grow and create pressure or discomfort. Many men do not even know they have one until it starts causing pain.
S***matoceles are benign, but when they hurt, they can be fixed with a straightforward outpatient procedure called s***matocelectomy. We isolate and remove the cyst while preserving normal testicular function.

Low testosterone isn't one-size-fits-all. Age changes everything.
At 28: The issue is rarely the testicles themselves; it's usually insufficient signaling (low LH/FSH from pituitary/hypothalamus). Clomid or HCG stimulate your own system to ramp up natural T production, preserving fertility and avoiding shutdown.
At 55: Testicles often lose function with age (primary hypogonadism). TRT becomes the go-to to replace what's no longer made.
Young men: Don't default to TRT like older patients. Get the right eval and protect your natural axis!

Normal semen analysis but recurrent miscarriages (even just one)? The missing piece: s***m DNA fragmentation (DFI).
Standard tests check count/motility/morphology—but not DNA integrity inside the s***m. High fragmentation from oxidative damage (common causes: varicocele, smoking, ma*****na, alcohol, lifestyle) impairs embryo formation → higher miscarriage risk.
It's an advanced test beyond basic semen analysis. If you've had losses with "normal" results, test DFI—many men discover treatable issues.
