Dr. Kian has contributed to media outlets for his cutting edge, aesthetic techniques and has been recurrently featured on CBS, The Doctors TV and Extra!. Recognized as a pioneer in non-invasive procedures, Dr. Kian personally trains surgeons across the nation on best practices of PDO thread techniques, microcannula filler injections, and utilization of autologous blood plasma for aesthetic and reconstructive purposes. He also plays a role in research and development as the Medical Director of CosmoFrance since 2012, an acclaimed medical manufacturing company. “People are always shocked when I tell them I’ve had my nose done… They say they can’t imagine me with any other nose, because this one fits my face so naturally. It feels like the one I was always meant to have.” — Hannah

When Hannah came to see me, her main concerns were a bulbous, overprojected tip, bridge deviation, and a nose that felt heavier and more dominant on her face than she wanted. She was also dealing with internal narrowing and weak structural support, the kind of issues that often show up as always favoring one side to breathe, waking up with a dry mouth, feeling “stuffy” and assuming it’s just allergies, or getting used to poor airflow because it’s been your normal for years.

Subtle work > obvious work.

If your side profile feels heavy or your eyes always look tired in photos, this is the kind of result people are actually asking for in 2026.

In this case, my patient’s main concerns were a droopy tip, bridge bump/unevenness, and under-eye heaviness that created a more fatigued appearance than intended.

These are some of the most common reasons patients seek consultation. Most are not trying to look like someone else, they want their profile to feel cleaner, their eyes to look more rested, and their features to feel more balanced.

I treated this patient with an Ultrasonic NoseTune® Rhinoplasty to de-project the nose, smooth and straighten the bridge, and refine the tip, while preserving enough bridge structure to maintain a natural, masculine aesthetic.

Lower Blepharoplasty with Fat Repositioning was also performed to reduce puffiness and soften the hollow under-eye transition, since that area can appear more pronounced once the nose is refined.

Today’s patient case highlight is a good example of what I call an ✨Ultrasonic NoseTune rhinoplasty✨

This patient wasn’t looking for a completely different nose, she wanted refinement. Her concerns were a drooping tip, a small bump along the bridge, and fullness through the tip that felt out of balance.

When I evaluate a case like this, I’m not just looking at each concern on its own I’m looking at how they’re connected. Because what feels like one issue is often a combination of small structural imbalances.

Using an ultrasonic approach, I refined the bridge in a controlled way, softened areas where cartilage was creating visible edges, and made conservative adjustments to tip support. At the same time, correcting the nostril position helped improve symmetry and proportion through the lower third of the nose.

This is where a lot of patients get surprised. What people think is a “tip lift” usually isn’t about rotating the nose upward, it’s about improving the underlying structure so the tip sits where it’s supposed to. Once that support is corrected, the nose naturally looks more refined and balanced without looking overdone.

That’s really what a NoseTune is: small, intentional adjustments that bring everything back into harmony.

There’s a lot that goes into being a good doctor. And then there are people who take it a step further…

Dr. Karimi is one of those people.

He’s in the operating room, yes, but just as much of the work happens outside of it: going back through cases, refining technique, and teaching other providers how to handle complications and think beyond the procedure itself.

And that part matters.

Because the best in this field don’t feel the need to gatekeep, they share what they know, they teach it, and they raise the level of everyone around them.

Not every filler story ends the way you expect…

This beautiful patient Sheilah, came to me after developing a hyaluronic acid filler granuloma, and what followed wasn’t just swelling or irregularity, but a much more complex process beneath the surface.

After multiple rounds of ultrasound-guided treatment to dissolve and calm the area, the granuloma was successfully resolved. but what it left behind was significant tissue atrophy and volume loss from the chronic inflammation.

To restore what had been lost, I performed fat grafting with regenerative nanofat in-office under local anesthesia, focusing not just on replacing volume, but improving the quality and health of the tissue itself.

At just one month post-op, sheilah’s results already reflect true facial restoration restoring structure, softness, and balance. And for her, that meant finally feeling like herself again.

At her follow-up, Sheilah shared with me that her face finally feels restored… and with that, her confidence came back. In her words, it’s been “life-changing!” ✨
