Thank you to Dr. Justin Karlin for inviting me to speak at this incredible event and kudos to doing an amazing job as a co-chair of this illustrious course.  
 
The three of us standing together is symbolic - we represent our respective specialties of ophthalmology / oculoplastic surgery, otolaryngology (ENT) / facial plastic surgery and plastic surgery.  While our training background is diverse, our expertise often intersects for surgical and non-surgical intervention on certain anatomical areas of the eyes, nose, and face. 
 
In the second photo you see me with , the only RN on the faculty at this course over the past two years – I want to recognize Francine and all advanced practitioners who have taught me so much over my 15 year career – many don’t realize but it is our advanced practitioner colleagues including RNs, PAs, and NPs that are performing a vast majority of injectables here in the United States, and we can all learn new pearls from each other.

My lovely patient was looking for the right doctor for a while, and she traveled all the way from Houston, Texas to see me. She knew that she had ‘a big nose’ and that a lot of surgeons were eager to take on her case, but she was terrified of having her nose too upturned or to look like a ‘Barbie nose’ or a ‘Hollywood nose’. She took her time researching and ultimately decided to partner with me for her rhinoplasty because of my NoseTune® philosophy. This is my highly customized surgical plan tailored to each patient’s unique nasal structure and aesthetic vision to achieve harmony, balance, and refinement while preserving what makes each face unique.

She didn’t want to have too small of a nose that might not fit with her amazing bone structure, and once we met virtually and I showed her a 3D simulation of what we could achieve together, she knew that I was the right doctor for her. When she came out to LA, I noticed that she had some crookedness, her tip veered off to one side and drooped downward.

Today’s patient case highlight is a follow-up on one of my most memorable revision rhinoplasty patients — now one year post-op. She had lived with the results of a prior rhinoplasty she hated for over 20 years. A busy physician, she delayed revision for years while juggling medical training and family life. When we finally met, she had done extensive research, looking for a surgeon who could improve both her significant airway issues and the aesthetics of her nose — while respecting her Indian heritage and preserving her ethnic identity.

When we moved forward with surgery, I performed a complex revision rhinoplasty. We had a rib graft ready as a backup, but fortunately, because her initial surgery had spared her septum, I was able to harvest enough cartilage internally. The changes were comprehensive: improved nasal tip projection and symmetry, a straighter bridge, and restored breathing function. Now, one year out, she remains thrilled with the results. She says the transformation has been life-changing. Most importantly, she still sees herself in the mirror — she still looks Indian, and the result feels authentic, not “overdone.” That balance was our top priority.

Revision rhinoplasty patients often face a longer recovery curve than primary cases. At the one-year mark, most swelling has resolved — especially at the nasal tip, which can take the longest to settle. What you’re seeing now is a near-final result, with just a subtle degree of refinement still possible in the coming months. This case is a powerful reminder that with the right surgical plan and patient trust, it is possible to restore both function and confidence — even after decades of disappointment.
