This patient presented with a challenging combination of under-eye concerns—both a traditional lower eyelid bag and a secondary fullness known as a festoon or malar mound.

To achieve a natural and lasting result, used a multi-layered surgical approach:

✔️ Transconjunctival fat repositioning to smooth the tear trough without external incisions
✔️ Skin pinch excision just below the lash line, extended laterally to tighten the festoon
✔️ A mini cheek lift to support the midface and address deeper structural laxity
✔️ Fat grafting harvested from the inner knees, placed in the cheeks, brows, and temples to restore youthful volume and balance facial contours
