Approximately 40-50% of patients undergoing screening mammogram have dense breast tissue.

About 30-40% have heterogeneously dense (BI-RADS category C) and less than 10% have extremely dense (BI-RADS category D).

Dense breast tissue makes it easier to miss small masses on mammogram so a breast ultrasound MAY be useful. However, it may also lead to unnecessary additional imaging, biopsies and increased healthcare costs.

A sudden lump "down there" can be alarming. Bartholin's cysts occur when a gland opening gets blocked.

Not all lumps are Bartholin's cysts and not all Bartholin's cysts need treatment.

However, they can suddenly flare and become large and infected which can be excruciatingly painful and often require treatment.

Patients often think, "I got my pap, I'm good."

But a pap test is just cervical cancer screening. It does not screen for painful periods, heavy periods, PMDD, PCOS, endometriosis, incontinence, menopause, breast cancer, family planning...

You are more than your cervix.

I recommend an annual well person exam (pelvic and breast exam) to screen for issues (sometimes you are due for a pap and sometimes not).

In between that though, I see patients to further work up and treat issues uncovered during their annual or have developed between visits.

To clarify my recent roundtable with The Female Quotient® :

Contraception is thousands of years old.
Easy access to modern IUDs is relatively new.

When it comes to insertion of IUDs:
Some people have terrible pain.
Some people have no pain.
One experience does not cancel out the other.

I believe in individualized care and joint decision making.

Providers do not have unlimited time and resources and neither do patients which is why I think communication is key.

Ultimately though, I am talking head on the internet and you should be getting medical advice from your provider.

Being in your 40s can be confusing since you may have to worry about contraception AND peri-menopause. While the birth control pill has estrogen and progesterone, it's not the same as HRT. Especially during your placebo week, the pill may make your hot flashes worse.

Your pelvic floor deserves more than "just deal with it".

Pelvic floor physical therapy treats more than incontinence. It is used for treatment of pelvic pain, pain with s*x, postpartum recovery as well as incontinence.

Just like a well woman exam is more than a pap, pelvic floor physical therapy is more than Kegels.

There’s a lot of conversation right now about doctors charging administrative or membership fees and I get why.

But here’s what often gets missed:

This model isn’t about charging more.
It’s about creating a practice where you’re not rushed, you can easily access your doctor and avoid ER and urgent care visits. Your care is thoughtful, not transactional.

The current system uses metrics and efficiency based on numbers and not people. It pays large groups/corporations astronomically more than small practices.

So some of us have stepped outside the system so we can provide our patients the care we think is best.

It is not a good fit for everyone and I understand some people are angry about it but others understand the need and value. My fee is not astronomical but it does cover things that insurance does not cover but I think necessary like direct messaging, finding the lowest out of pocket labs, and in office blood draws. I also do ultrasounds myself in my office so results are immediate.
