We closed out the year with joy, celebration, and community at our last Live Well with Via Care! Thank you to our incredible senior patients for taking the time to join us at our wellness events. Your presence is what makes Live Well so special. ✨

At our Holiday Event, we enjoyed delicious food, festive cheer, and a lively round of bingo with amazing prizes. 💙🎄

We’re grateful for the opportunity to gather, connect, and support your wellness journey. We can’t wait to bring even more events to our community in the new year!

Meet Rebecca, one of Via Care’s Enhanced Case Managers image.png

Every day, she helps patients overcome the barriers that make managing their health hard — from arranging rides and coordinating specialist visits to helping patients use their Medi-Cal benefits and access food, housing, and other essential resources.

ECM is hands-on, personalized support that keeps patients from falling through the cracks. With someone by their side, patients can focus on what matters most: staying healthy and feeling supported.

The flu vaccine is the best way to protect yourself and your community. It helps prevent serious illness, reduces the risk of hospitalizations, and keeps those around you safer, especially young children, older adults, and people with chronic conditions.
Everyone 6 months and older is recommended to get vaccinated each year.

✔️ Programa de 6 semanas

This program offers:
