In patients with a prominent maxilla and a retruded chin, the perioral muscles often become hyperactive to compensate for skeletal imbalance. Excessive upper lip elevation contributes to a gummy smile, while the mentalis overfires to permit mouth closure when chin projection is lacking.

This case was treated by addressing the drivers, not just the appearance:
• Targeted Botox to the LLSAN to reduce excessive upper lip elevation
• Structural chin augmentation placed deep on bone to support the mentalis

By decreasing upper lip pull and restoring chin projection, muscular demand is reduced, allowing the lower lip and chin to relax naturally. The result is improved profile balance, reduced chin strain, and more harmonious smile dynamics — all without altering the nose or midface.

In-Office Upper Bleph & Lower Eyelid Skin Pinch (Post-op 1 Week)

Patients often come in bothered by upper eyelid hooding and skin bunching. An upper blepharoplasty is an elegant solution: through a hidden incision placed in the natural eyelid crease, redundant skin is removed to restore a cleaner, more defined tarsal show. This creates a brighter, more open upper eyelid while maintaining the patient’s natural eye shape and expression.

Lower eyelid crepiness is another common concern. While lasers and chemical peels can help with mild cases, patients with more significant skin laxity often benefit from a lower eyelid skin pinch. Performed through a camouflaged subciliary incision, this technique precisely removes excess crepey skin without disturbing the underlying fat pads, improving lower eyelid texture, tightness, and smoothness.
