Highlands Nursing and Rehabilitation We provide a warm, welcoming environment and highly individualized skilled nursing care. Our Care Team is specially-trained to work with seniors.

Located in Louisville, Kentucky, Highlands Nursing & Rehabilitation offers compassionate 24-hour Skilled Nursing care in our 154-bed facility. We listen, we understand, and develop personal therapy and care programs designed to maximize your abilities and life. This includes assisting with daily living activities and skills, such as balance training, medication management, IV therapy, and diabetes management. Should you need support services, know that we can arrange everything from mental health consultations to x-rays, lab work to podiatry services, as well as private home and hospice care.

Success Spotlight!
Charlene was recently picked up by our Physical Therapy team after noticing she wasn’t as active around the facility. Our goal was to help her regain strength and endurance so she could feel more energized and engaged in daily life. Charlene embraced the challenge wholeheartedly—showing up in the gym every day with determination and a positive attitude.

Through consistent effort and the guidance of our dedicated therapy staff, Charlene has made remarkable progress. Today, she is moving confidently throughout the facility and participating in more activities than ever before. Even better, Charlene reports a noticeable decrease in fatigue since starting therapy, which has given her a renewed sense of independence and joy.

Her success is a testament to what can be achieved with hard work, perseverance, and a supportive care team. Congratulations to Charlene and everyone who helped make this milestone possible!
