SHC Medical Partners was formed in 2003 in response to the need for skilled nursing facilities (SNF) to have more frequent, on-site medical services. Today, we are a premier transitional medical services company that brings personalized medical care to those patients we serve. We currently provide medical services in 7 southeastern States and are very excited to be expanding our coverage area and

adding new service lines! Our dedicated and compassionate team is committed to building a trusting and rewarding relationship with those we care for and work with by providing our services with integrity and serving with humility. is moving toward outcomes-based reimbursement models and access to affordable insurance coverage. The demand for medical providers is further increased by the emergence of 71.3 million aging baby boomers and resulting medical advances that enable quality living through management of chronic diseases. The Primary Care provider remains the sole trusted authority for coordinating quality care, yet the shortage of physicians is at a critical level. The article, “Direct Primary Care: Key Issues and the Future” by Dave Chase, stated this primary care physician shortage is estimated to be 35,000 to 44,000 by year 2025 (Forbes, July 2013). We believe that these realities create an historic opportunity for SHC Medical Partners which is to respond to this growing need by providing transitional medical care, a model that is deeply embraced by our medical providers. We focus on delivering quality transitional medical care beginning with coordinating discharges from a hospital stay, proceeding to managing medical care while at rehabilitation facilities – with a goal of enabling our patients to ultimately reside in a setting they choose with proper medical oversight. Our medical providers participate in the full spectrum of medical service delivery including acute care, emergency care, rehabilitation care and primary care. Today, we celebrate National Hire A Veteran Day.

In November 2019, we set a goal of hiring 2,500 veterans by 2025. By early 2023, we met our goal and doubled it to 5,000 veteran hires in the same time period. We are nearly 90% towards this expanded goal.
