This week we’re proud to highlight Amanda McGrath, MA – Chronic Care Management Coordinator 💙🩺

What does CCM mean to Amanda?
-To her, Chronic Care Management means being a true advocate for patients and their families. She loves making a difference between office visits—helping patients manage their conditions, find resources, and stay on track with their care.

What drew her to CCM?
-With over 15 years in the medical field, Amanda has done it all—but CCM stood out because it blends her clinical knowledge with patient education, care coordination, and advocacy. Basically… the perfect fit. 👏

What does she love most?
-Building trust over time. Amanda says there’s nothing more rewarding than seeing patients become confident and comfortable reaching out—and when they recognize her just by her voice? That’s when you know it’s real. 💙

Her secret to making a difference?
-Consistency and always following through. It sounds simple—but it means everything to our patients.

Today is World Tuberculosis Day 🩺

On this day, we recognize when Dr. Robert Koch discovered the bacteria that causes tuberculosis—changing medicine forever.

While TB may not be something we see every day here, it’s still a serious health concern worldwide. Early detection, proper treatment, and awareness make all the difference.

Closer to home, tuberculosis had a major impact on our own community. Waverly Hills Sanatorium in Louisville was built in the early 1900s specifically to care for TB patients. Before effective antibiotics existed, patients were treated with rest, fresh air, and isolation—many staying for months or even years. It stands as a powerful reminder of how serious this disease once was and how far medicine has come.

✨🌟 PROVIDER SPOTLIGHT FRIDAY 🌟✨

MEET TIFANIE DONINGER, APRN 👩🏼‍⚕️🥼🩺

I have been an APRN since 2016, with the entirety of my experience focused in a house call practice. I chose to become a primary care provider because I am passionate about helping others and making a meaningful difference in their lives. I treat each of my patients as if they were a member of my own family, ensuring they receive the highest level of compassionate, personalized care.

Providing care in the home setting allows me to support patients who are unable to leave their homes, meeting them where they are most comfortable. This approach not only improves access to care but also allows for a deeper understanding of each patient’s environment and needs.

My philosophy of care is simple: I would never recommend a treatment plan that I would not feel confident providing to my own family. I believe in thoughtful, patient-centered care built on trust, respect, and clinical excellence.

In addition to primary care, I have developed a strong interest in wound care. I find it especially rewarding because, unlike many chronic conditions, wounds can heal with the right treatment and consistency. Being part of that healing process and seeing tangible improvement in my patients is incredibly fulfilling. I look forward to continuing to grow in this area and serve more patients in need of wound care.

Fun Facts:
• I love traveling, hiking, and spending time outdoors—I truly believe sunshine and fresh air are some of the best forms of medicine.
• My go-to drink is ice-cold lemon water.
• My favorite comfort food is vegetable soup.
• My favorite snack is sweet and spicy pickles.
• My current favorite show is Landman.

When I’m not caring for patients, I enjoy spending time with my children and cheering them on in all of their extracurricular activities.

👩🏼‍⚕️ Why primary care? - I chose primary care because it allows me to care for patients as whole people over time. After more than 10 years as a provider working in urgent care, palliative care, and primary care, I found that primary care best aligns with my values. Being able to individualize care, focus on prevention, and support patients through all stages of life—including serious illness—has been challenging at times, but ultimately the most rewarding and fulfilling work I’ve done.

🏡🩺 What do you love about home based care? - Home based care allows providers to see patients in their most comfortable environment. In my experience, this allows patients to be more open and honest in conversations about their health. It allows patients to have lower stress in regard to logistics of appointments such as getting ready for appointments or finding transportation. Appointments are more personal and flow easily.

📣🗣️ One thing you wish patients knew? - We think about you more than you know. When the work day ends and we start evening routines, it’s often hard to remove ourselves from the struggles of the day.

❤️‍🩹 What is your approach to care in one sentence? - My approach to primary care integrates a palliative focus, prioritizing comfort, symptom control, and proactive care to help patients remain safely at home and out of the hospital whenever possible.

🩹 Why did you choose wound care as your specialty? - I like wound care for a few reasons: it’s challenging, but each patients wound is its own unique puzzle that you have to figure out. I like the fact that I can actually cure an issue, there’s an end result. And lastly, there’s a huge need for quality home-based wound care.

That night, he defeated Sonny Liston to become the heavyweight champion of the world — and Louisville’s own never looked back.

Ali wasn’t just a boxer. He was resilience. He was discipline. He was heart.

And here’s something that makes this especially meaningful for us in Louisville:

Later in life, Ali lived with Parkinson’s disease for more than 30 years. He used his platform to bring awareness to neurological conditions, research, and compassionate care. His strength didn’t end in the ring — it showed up in doctor’s offices, treatment plans, and everyday battles that don’t come with a spotlight.
