Hey Everyone! Brooks Family Medical is a growing primary care house call practice dedicated to helping elderly patients age in place. We are seeking a Behavioral Health Care Manager (BHCM) to support our Collaborative Care Model (CoCM) behavioral health program. The BHCM will work closely with primary care providers and a psychiatric consultant to provide comprehensive, patient-centered mental health support to older adults with conditions like depression, anxiety, and dementia. Hi Everyone,

Joseph Brooks here. I am so happy to share with you all what I have been working on for the past 6 months. After 10 years working in the Primary Care House Calls business, I was blessed with an amazing opportunity to open my own practice. I have a physical office located in Harbor House of Louisville where I get to serve their participants at their Intergenerational Life Center. These are mostly individuals with intellectual and developmental disabilities, but will soon be expanding to include pediatric and geriatric participants. (If you are not familiar with what Harbor House Louisville is doing over there, you need to go check it out ASAP. They are on to something very special and life giving.) In addition to serving the participants at Harbor House, I am now open to the community as well. So spread the word and come see me.
