On March 31, 2025 Medicare coverage for outpatient speech, occupational, and physical therapies via telehealth will no longer be available. This negatively impacts individuals who need services over the computer due to many factors. Some of these factors may include:

Individuals that:
-live in rural or underserved areas
-cannot drive to outpatient clinics due to medical problems or cannot afford the transportation
-cannot find specialists or therapists who service their specific conditions in their local areas
-have acute conditions such as a recent stroke and cannot be on a clinic waitlist for 6 weeks.

If this doesn't impact you now, it will likely impact you or someone you know if not now, but down the road. Telehealth is critical for access to care- strokes, head injuries, chronic pain, etc. Additionally, if you don't have Medicare, private insurers (including advantage plans) and Medicaid tend to follow suit with Medicare's guidelines. Therefore, telehealth access will likely be cut for others down the road if Congress doesn't act. Congress meets again on March 14th, so please send letters or call your representatives letting them know how telehealth access for Medicare beneficiaries has impacted you or someone you know in a positive way.

I was very intimidated by AAC as a new therapist 10+ years ago. One thing that helped me in my journey is accepting cases that I was scared of. Those were the ones I made some mistakes with, but learned the most with. They forced me to get out of my comfort zone and collaborate with other disciplines as well. I now provide consulting for early intervention, home health, and have my own practice specializing in AAC, so it can be done!
My vision is to empower therapists or families needing devices so they feel confident in navigating the trials, funding, and evaluation process.

We hear a lot about “presuming competence” in the AAC world. Presuming competence means there are no prerequisite skills required before you can provide someone access to an AAC system. If we believe this, we make the least dangerous assumption.

Every individual we work with will have their own abilities and needs that require us to explore what those are and determine the best route or multiple routes for accessible communication. It is important to keep an open mind and positive perspective about initiating an AAC system, while presuming the individual needs to communicate, learn, and connect with those around them. The client’s needs, however, will determine the type of AAC or access method. We provide them with opportunities, time, data, and use evidence-based decision making to determine the type of AAC and access method.

The slide that lists “Challenges” to this “Presuming Competence” framework are meant to bring awareness and explain why this is not always being implemented, not to justify why we wouldn’t use it. The aim should be to provide opportunities that align with the individual holistically so they can succeed and access language.

✍🏼Peak Speech Therapy’s goals for 2025- It is easy to get distracted from the purpose and vision behind social media posts. I believe it is important to intentionally realign yourself as to why you started a project- check in to make sure it still feels right. In 2024, I stepped out of my comfort zone to start scaling my business socially. I’ve always been interested in the education piece behind our field and had an idea for creating educational and accessible speech-therapy materials for those who want to learn about alternative and augmentative communication and specifically those with Primary Progressive Aphasia- a complex diagnosis that can often get left on the back burner behind aphasia. In 2025, I hope to reach more people with PPA, create connections among those individuals, and provide resources for those to confidently navigate AAC.

What do those with PPA and their loved ones want from speech therapists? A research article by Loizidou et al. (2023) sought to answer this question. Common themes emerged, including the communication partner’s journey, adjusting to the diagnosis, communication abilities and difficulties, and challenges beyond language (International Journal of Language & Communication Disorders).

Providing communication partner training is one area which can assist communication partners on ways to help. Good conversations that are balanced help people feel respected, allow both parties to communicate ideas and feel as though they were heard. Additionally, using these well known communication strategies allow the person with aphasia to make decisions for themselves, self advocate, and meaningfully connect.

Selecting the wrong access method for an AAC user can compromise the accuracy of their performance with their AAC device and language system. Access methods require their own set of skills, separate from language use. For instance, if a child is using an access method that does not align with their physical needs, it may hinder their ability to effectively engage with and use their language system.

The Assessment of Learning Process (ALP) helps you:
✅ Break access method learning into clear, observable phases and stages.
✅ Focus on BOTH access methods and language separately.
✅ Set goals based on actual progress, not assumptions.
