Job description
The Director of Nursing is the Clinical Manager as defined in 484.105(c) as per Conditions of Participation and, also serves as the Supervising RN as defined in 97.243 in the Texas Administrative code. The Director of Nursing [Clinical Manager] is responsible for the performance of clinical nursing staff in producing optimal clinical outcomes for patients. The Director of Nursing [Clinical Manager] assures that best practices are utilized to deliver effective, timely, and compliant skilled nursing and other skilled clinical services, performed under the Licensed and Certified and Licensed Home Health Services categories of licensure. The Director of Nursing [Clinical Manager] will assure the clinical staff employs best practices and focuses efforts as a team toward optimal patient outcomes.
Supervision of Clinical Personnel: The Director of Nursing [Clinical Manager] is responsible for:
developing and mentoring nurses to achieve optimal patient outcomes through the following systems:
OASIS assessments, CLASS IDRC assessments where appropriate, Appropriate ICD Coding assuring that development and administration of the Plan of care includes input and collaboration by all appropriate disciplines to assure services are provided at optimal levels allowed under the Medicare or other health care program, to achieve optimal patient outcomes.
assuring appropriate safety and infection control measures are implemented to protect patients and staff from disease or injury, including but not limited to training, supplies, personal protective equipment and processes.
working closely with the Program Director for LHH and PAS programs to assure that all requirements for LHH [CLASS] nursing are met with clinical competence.
providing backup for clinicians
Compliance:
The Director of Nursing [Clinical Manager] is responsible to assure that nursing and other clinical therapeutic services are delivered according to the licensing requirements for Home and Community Support Services Agencies (HCSSA’s), as well as the guidelines of the program under which the client was admitted, such as the Conditions of Participation and Conditions for Coverage of Medicare services, and according to professional standards. The Director of Nursing [Clinical Manager] assures that:
patients are accepted for treatment on the basis of a reasonable expectation that the patient's medical, nursing, and social needs can be met adequately by the agency in the patient's place of residence. patients qualify for services based on the requirements of the program (Medicare, Medicaid, or other third-party payor) under which their care will be rendered. The Director of Nursing [Clinical Manager] or Alternate Director of Nursing [Clinical Manager] must: be available to agency personnel at all times in person or by telephone; participate in activities relevant to services furnished, including the development of qualifications and assignment of agency personnel;
ensure that a client's plan of care or care plan is executed as written; and
ensure that an appropriate health care professional performs a reassessment of a client's needs: Caprock Home Health Services, Inc Position Description revised 01/2018 when there is a significant health status change in the client's condition; o at the physician's request; or o after hospital discharge. The Director of Nursing [Clinical Manager] will work with Intake personnel as well as physicians to assure that all documentation required for billing is completed and incorporated into the record in a timely fashion and that billing is not completed until the Plan of Care and Face to Face are properly completed and documentation is in file. The Director of Nursing [Clinical Manager] is responsible to assure that a written summary report for each patient is sent to the attending physician at admission and at least every 60 days as required. The Director of Nursing [Clinical Manager] assures that nurses working in the Licensed and Certified programs of the Agency conform to the Medicare Conditions of Participation
Quality Management:
The Director of Nursing [Clinical Manager] assures that Quality Assessment and Performance Improvement Activities are conducted as required to assure appropriate and effective nursing and other therapeutic services to clients in all Licensed Home Health and Licensed and Certified Programs.
Fiscal Responsibility:
The Director of Nursing [Clinical Manager] will work with Administration and management to effectively manage the budget for their department setting appropriate levels of care by applying ICD-10 codes and HHRG recommended levels when assigning visits per patient and where possible assigning the most qualified clinician based on expertise in medical condition, and planning the schedule so that travel is minimized in order to optimize clinician time and foster efficiency in visits.
The Director of Nursing [Clinical Manager] cooperates with staff members in carrying out the overall objectives of the Agency, and may be called upon to perform other Duties as assigned by the Administrator, Managers, or Executive Vice President.
Job Skills and Abilities:
Must possess comprehensive knowledge of general nursing theory, technique, and practice plus ability to effectively work with various clinical disciplines and personnel. Must have a comprehensive understanding of the OASIS data set, its purpose, use, and processes for completion. Must have the ability to research issues and to use and incorporate information and materials researched to assure understanding of rules, policies and processes. Must have/develop basic competency with ICD 9/10 coding sets, theory, processes and use. Must have propensity for problem solving. Ability to learn, manage, and ensure compliance of various health programs, including but not limited to Medicare Conditions of Participation and Medicare Conditions for Coverage of skilled services, THHS HCSSA licensing regulations, and THHS and/or Texas Health and Human Services (HHSC) program nursing requirements. Strong understanding of Quality Assessment and Performance Improvement (QAPI) techniques and procedures. Must have the ability to lead a team, evaluate performance, organize processes and personnel, and apply regulations and policies in various situations and behaviors, working with a variety of personnel with various working and learning styles to maintain compliance, discipline and productivity. Must learn and become proficient with point of care software and pathway tools. Practices in compliance with the regulations of the Texas Board of Nurse Examiners. Strong written and oral communication skills required.
