We are looking for 2 experienced Registered Nurses who can work independently and make decisions on how to care for patients. The RN will be responsible for the assessment, planning, implementation and evaluation of care in a home health setting. Responsibilities will include creating a full treatment plan, supervising and training lower level health care professionals, and identifying opportunities for clinical growth. The ideal candidate has a background in Home Health services and has experience working with Orthopedics, Post-Surgical, and Wounds.
Responsibilities:
Be able to work in a variety of home health settings.
Communicate effectively with patients, family and colleagues.
Initiate appropriate preventive and rehabilitative nursing procedures.
Assume responsibility for coordination of care.
