“Back Mice” – A Common but Overlooked Cause of Low Back Pain.
Also known as episacroiliac lipomas, “back mice” are small, firm, movable fatty nodules that develop in the lower back—usually near the top of the hip (iliac crest). These nodules occur when fat pushes through the connective tissue (fascia), creating tender spots that can be surprisingly painful.
Common Symptoms:
• Small, tender, movable lumps in the lower back
• Localized low back or sacroiliac pain
• Pain that may radiate into the buttocks or thigh (often mistaken for sciatica)
• Increased discomfort with sitting, standing, or walking for long periods
Who’s at Risk?
They tend to be more common in women and may be associated with strain, trauma, or chronic wear and tear on the lower back.
Treatment Options:
• Manual therapy (including deep tissue massage)
• Targeted injections for pain relief
• Heat therapy or anti-inflammatory support
• In persistent cases, minor surgical removal
