Autumn Fields Senior Living 20 bed facility for people 55 years of age or older offering care for early onset Alzheimer’s/dementia. Both men and women may choose to live at Autumn Fields.

Autumn Fields is a 20 bed Assisted Living Community offering living, recreational and kitchen/dining space. The home is located in Luxemburg, Wisconsin with access to community activities and services. Autumn Fields will be home to a maximum of twenty adults who have mental or physical handicapping conditions, AA-Advanced age, ALZ- irreversible dementia/ Alzheimer's, TBI, TI- terminally ill. Resid

ents who have had traumatic head injuries may also be served as well as hospice services as needed. The home will provide private rooms with handicap assessable private bathrooms for all residents. The focus of Autumn Fields in the future will be for adults suffering with traumatic head injuries or memory impairments (Alzheimer's/Dementia).
