Look at that shoulder range of motion! 👀

Let me tell you a little bit about Terry…

He had to make quite the commute to PT but always showed up early to his appointments and ready to work. 🚘

Halfway through his shoulder rehab, Terry went on vacation. Instead of coming back to PT with lost gains, he returned with more motion because he stayed consistent to his home program. 🫵

When Terry had the mobility and strength to do simple tasks, he wasn’t satisfied. He had the goal of being able to get on the golf course with no restrictions. And guess what…he did. ⛳️

So excited of all the progress Terry made after shoulder surgery! Proud to be his PT.

⚡️ Radial Nerve Sliders for Arm Pain and Numbness

🎯 The aim is to mobilize the peripheral nervous system for a reduction of pain, numbness, and tingling by means of decreasing threat perception.

Conditions in which neurodynamics for the radial nerve can be helpful:

✅ Cervical Radiculopathy
✅ Thoracic Outlet Syndrome
✅ Radial Tunnel Syndrome
✅ Posterior Interosseus Nerve Syndrome

🖐 Above I demonstrate a progression beginning with radial nerve sliders and then tensioners that are options for treatment management. Each variation mobilizes the nerve from neck to fingers; however, the difference is that tensioners place a greater stretch on the nerve.

☝️ NOTE: Make sure to depress the scapula with the unaffected arm prior to performing each mobilization for appropriate technique.

⚠️ It is important that these do not cause a lasting increase in symptoms. If this is the case than intensity and load need to be modified.

Myra came to see us for persistent neck pain that didn’t resolve with previous physical therapy. She received multiple imaging studies that didn’t reveal anything requiring surgery. After evaluating Myra, we found that she was suffering from debilitating neck pain and headaches because of weakness in her shoulder from a previous surgery. The underlying reason for her neck pain/headaches wasn’t treated before, thus her symptoms did not improve.

After much dedication and consistency to her PT, Myra’s neck/headache symptoms have resolved and now she has the goal of being able to reach overhead with her operative arm. So thankful that Myra trusted us with her care! 🙏

Electrical Dry Needling to the Upper Trapezius 👀

The Upper Trapezius is a very common area where we carry stress and can be the home for myofascial trigger points. Dry needling is a safe and effective intervention as it decreases pain/headaches, reduces abnormally high muscle tone, and improves neck/shoulder mobility. 👌

🍑 When strengthening the glutes there tends to be a focus on the sagittal plane (backwards) and for good reason. This targets the glute max which not only helps aesthetically but also is a powerful hip extensor that improves functional performance.

🤔 In contrast, there is less attention geared to the frontal plane (side to side). As more research comes out there can be a strong argument made that this plane is just as much if not more important to work in.

❓Why? The simple answer is pelvic stabilization. Taking a deeper dive on this topic shows that having the ability to stabilize the hip and pelvis in the frontal plane allows for proper alignment and sets the rest of the lower kinematic chain up for success by providing a solid base to work on.

🤕 In addition, being strong in the frontal plane decreases the risk for lower extremity injuries such as an ACL rupture, IT band syndrome, and patellofemoral pain syndrome to name just a few.

💯 The primary muscle responsible for pelvic stabilization is the gluteus medius. Therefore, improving the recruitment and strength of this muscle could pay huge dividends in regards to performance ability and keeping an athlete between the lines instead of outside in the training room.

👆 See above Side Plank with Hip Abduction (dominant leg on bottom) which has been shown to produce the highest percentage of maximum voluntary isometric contraction (MVIC) of the glute medius.

👨‍⚕️ Achilles tendinopathy is an overuse injury of the Achilles tendon due to the gradual irritation of tendon tissue from repetitive loading. Dry needling is often an intervention we use at Dockery & Associates PT (along with stretching/strengthening exercises, soft tissue mobilization techniques, and joint mobilizations) to help treat this condition. Needle placement can be targeted at the Achilles tendon insertion, Achilles tendon mid-belly, and gastroc-soleus complex to facilitate the healing response and restore normal muscle length-tension.
