CMS’ new National Provider Directory won’t replace NPPES, but it adds what NPPES can’t: real affiliations, FHIR endpoints, and near real-time updates.

We just closed the 2025 ACO submission season — and our clients delivered.

Across our portfolio, Healthmonix ACOs outperformed the national Medicare CQM average on 2 measures where a 2024 benchmark exists.

The other is a new 2025 measure with no standard to compare against:
📊 HbA1c control (001SSP): 73.4% — 6 points above the national average
📊 Blood pressure control (236SSP): 71.0% — 10 points above the national average
📊 Breast cancer screening (112SSP): 65.3% — new measure, no prior benchmark

Our top performer hit 96.8% on diabetes control across a patient population of almost 200,000. This type of performance happens when clinical teams see their gaps in real time — not after the performance year is over.
