Cochran Internal Medicine & Endoscopy Partners with Main Street Health to Bring Extra Resources to Patients in Mammoth Spring
Cochran Internal Medicine & Endoscopy has announced an exciting new partnership with Main Street Health aimed at providing an extra layer of support to patients in its Mammoth Spring Clinic.
Dr. Christopher Cochran is a local primary care physician who has proudly cared for patients in southern Missouri and northern Arkansas for the last 20+ years. Main Street Health is a value-based care company focused exclusively on serving patients in rural America.
Dr. Cochran and his clinic staff have welcomed Arieona Smith, Main Street Health’s Navigator to its team. Arieona will serve as an extra set of hands to clinic staff, connecting patients with essential community resources and supporting patients with their personalized care plans.
The Health Navigator will support the practice with several activities including:
- Coordinating care for medically complex patients
- Connecting patients to community resources and assistance programs
- Helping patients manage chronic conditions
- Ensuring follow up after hospital admissions
- Educating patients on their insurance coverage and benefits
- Assuming administrative tasks so that providers can spend more time with patients
Dr. Cochran stated, “Arieona's expertise and gentle, caring attitude with patients has greatly improved communication between patients and staff. We are grateful to have her join us in supporting the community.
