📢 ND Medicaid Update – Therapy Visit Limits Changing in 2026

North Dakota Medicaid has announced an important update affecting therapy services for members age 21+, effective February 1, 2026.

Medicaid is shifting from per-visit limits to per-unit limits for therapy services:
• Physical Therapy: 60 units (previously 30 visits)
• Occupational Therapy: 60 units (previously 30 visits)
• Speech Therapy: 30 units (previously 30 visits)

🔹 Claims will be evaluated based on units billed, not visits
🔹 Services beyond these limits will require prior authorization
🔹 Only the updated SFN 481 authorization form will be accepted starting Feb. 1, 2026

Even if you don’t provide therapy services directly, this may impact referrals, coordination of care, or downstream billing workflows.
