Hello everyone!

We are reaching out to the communities we serve in an effort to continue to offer low cost, high quality health care services at DeSoto Healthcare Center, Inc. We are participating in the Give for Good fundraising event this year. We are asking for donations to assist us in continuing to provide affordable health care to individuals who are uninsured and live below the poverty guidelines. We provide office visits for patients for as little as $5-$40 for those who qualify on a sliding fee scale. Please help us to continue to meet these needs. The ACTUAL Date is May 3, 2016. Hello all,

Just a few announcements on what DHC has to offer for the public in the upcoming month. As always we are seeking innovative ways to improve access to health care that is affordable, comprehensive and high quality. Our goal is to improve health outcomes by increasing health care awareness.

SMOKING CESSATION WORKSHOP

Thursday, January 29, 2015, Project P.U.S.H. will be sponsoring a free health care workshop on smoking cessation at DeSoto Parish Council on Aging from 1:00 p.m. until 2:00 p.m. The workshop is free and open to the public. Informative information regarding to***co use cessation and the adverse effects to***co use has on the body and quality of life.

MARKETPLACE INSURANCE ENROLLMENT WORKSHOP

The deadline for enrollment for the Affordable Care Act Marketplace Insurance is February 15, 2015. In an effort to reach out to individuals in the area we are collaborating with the Martin Luther King Health Center and Pharmacy to present an opportunity for enrollment. We would like to invite the public to join us at DeSoto Healthcare Center on Friday, January 30, 2015 from 1:00 p.m. until 5:00 p.m. and Saturday, January 31, 2015 from 9:00 a.m. until 1:00 p.m. We will be providing enrollment assistance to all interested in enrolling prior to the deadline. Hot Dogs and other snacks will be available on Friday and Saturday with door prizes awarded for participants. Please stop by and apply for enrollment.

MEDICAL MOBILE SCREENING HEALTH FAIR

A collaborative effort to provide health screening in the DeSoto Parish area between St. Luke's Episcopal Medical Ministry, Feist Weiller Partners in Wellness, Martin Luther King Center, Susan G. Komen of Northwest Louisiana and DeSoto Healthcare Center will culminate in Walmart parking lot on Tuesday, February 10, 2015 at 9:00 a.m.
Free screenings include:

BREAST EXAMS
MAMMOGRAMS
BLOOD PRESSURE SCREENINGS
BLOOD SUGAR SCREENINGS

DeSoto Healthcare Center invites the public to take advantage of these free services brought to the area to improve the health of individuals in and around DeSoto Parish.

HAPPY HOLIDAYS!!!

We at DHC hope you are having a Happy and Healthy Holiday season! We are looking forward to the New Year as we improve on current services and expand to include additional services.

We will be launching our new DHC website in January 2015. Look for us as we provide a more detailed, patient friendly, informative, website which will inform all those looking for a comprehensive, high quality, health care facility where the patient is the focus.

Also, check us out at www.nlachronicdisease.org as we plan to expand our efforts to decrease health disparities by proactively addressing chronic diseases throughout north Louisiana.

In order for all staff to enjoy fellowship and family this holiday season, we will be closing at 12 noon on Wednesday, December 24, 2014 and will reopen for regular business hours on Monday, December 29, 2014 at 8:00 a.m. We will also be closed on Thursday, January 1, 2015 and will resume regular office hours on Friday, January 2, 2015 at 8:00 a.m. If you feel you have a life-threatening emergency, please go to your nearest emergency room. Just letting you know that DHC will be sponsoring a "Fall Back into Healthy Habits", OPEN HOUSE on November 20, 2014 from 9:00 a.m. until 4:00 p.m. Lots of activities including B/P screening, vision and hearing screening, food, door prizes and educational resources. Open to the public. Come out and join us. If you have not visited us before, here is an opportunity to see what health care services and resources we have to offer. If you have visited us before, come by and see what's new.

Hello all,

To all who have not already came in for their influenza vaccines-They are available and going fast, please schedule a time to come in and get your vaccination or just stop by. We accept walk-ins, have same day appointments or by appointment call (318) 871-1633 or schedule appointment on line.

To all who have not been introduced to the Marketplace Insurance or to those who are aware and need additional information. DeSoto Healthcare Center will be sponsoring workshops for the next 2 weeks in preparation for open enrollment which begins on November 15. Check your local communications channel and weekly local paper for locations, dates and times.

These workshops will be informative, interactive and allow the community to be aware of the resources available to them to assist in obtaining insurance. DeSoto Healthcare Center has a full-time Certified Application Counselor to assist with enrollment available on site. Call and schedule appointments. Remember there is a penalty which is required by the government for individuals who do not attempt enroll. Even if you do not qualify, enrollment applications will prevent you from having to pay the penalty.

DHC is also sponsoring free health maintenance and illness prevention workshops throughout the community through Project PUSH. Chronic disease topics include diabetes, high blood pressure, weight management, cholesterol disorders, and smoking cessation. Please contact the office if you are interest in attending our group therapy on site or a community event.

We at DeSoto Healthcare Center would like to thank our patients and invite those in DeSoto parish to stop by and visit our facility. We provide comprehensive care services with a qualified, friendly, professional staff to meet your health care needs. We are available six days a week, Monday through Saturday, with extended hours on Tuesdays from 9:00 a.m. until 6:00 p.m. and on Saturdays from 8:00 a.m. until 12:00 noon.

We have discount office visits for qualifying patients as low as $25; Discounted lab prices and a 30% discount on office visits for private pay patients paying on the day of the visit. We accept most major insurances, Medicaid and medicare.

Hello all,

Just wanting to say get prepared for flu season. Immunizations should be available by mid-October. We will offer them for $25.00 to everyone. Of course, if you have insurance, Medicare or Medicaid, we have you covered as well. Don't get caught in the season without proper vaccinations.
