INTRODUCING
TELE-MEDICINE



Due to the unprecedented state of our society. Mansfield Urgent Care and Family Medicine has implemented our Tele-Medicine Application. This allows you, our patients, to have virtual medical visits from the comfort of your own home. Please call our office for details: 817-473-3979.

Dr. Fisher’s staff can help determine if Tele-Medicine is the right type of visit for your needs. Our staff will schedule your appointment time and aid in the setup of your patient portal.

After our office gives you your individual username and password the portal set up instructions are:

Visit : www.YourHealthFile.com from any web-enabled device.
Click the "Activate your account here" button above the User Login form. An Account Activation screen will display.
In the "Username" field, enter: your unique username given to you by the clinic
In the "Password" field, enter: your unique password given to you by the clinic
In the "Patient’s Date of Birth, enter the patient's date of birth.
Click the "Activate Account" button. Upon confirmation, the Update Account screen will display and you will be prompted to enter your desired account details.
Complete the Update Account form, then click the "Update Account" button.
As always, our patients are welcome to be seen in person at the clinic. We are taking every precaution to make the facility safe for our patients. We ask any patient with virus-like symptoms to call the clinic in advance. We will meet and diagnose these patients outside the clinic.

Given the reduced volume of sick patients, we may temporarily shorten our evening hours. Please call ahead if you are interested in being seen after 7:00PM weekdays. Please call the clinic with any questions, suggestions, or concerns. Please do not reply to this email as we will be unable to respond back.
