ADMH (formerly the Adult Down Syndrome Program), provides comprehensive, compassionate, coordinated, patient-centered care within the framework of a Patient Centered Medical Home. We provide a holistic approach to healthcare that includes medical, pharmacological, psychological, nutritional, behavioral and social approaches to care by a team of providers who put the patient and their families/caregivers at the center of their healthcare needs.
