I am seeking a patient model for an exclusive training program in Des Moines, IA.
This is a unique opportunity to receive a COMPLIMENTARY full-face, on-label treatment using both dermal filler and neurotoxin.

💉 Details:
• Treatments will be complimentary
• No additional compensation or travel/expense reimbursement
• Patient must sign consent forms, including before & after photo consent for educational and marketing use

👤 Model Criteria:
• In good health
• Good skin quality with minimal sun damage or hyperpigmentation
• Presenting moderate to severe dynamic wrinkles and folds in two or more of the following areas:
A. Nasolabial folds (NLF)
B. Indirect NLF Correction
C. Submalar wrinkles/folds (radial cheek lines)
D. Perioral rhytids (vertical lines/wrinkles around the mouth and lips)
E. Marionette lines (smile lines)
F. Oral commissures (corners of mouth)
G. Mental crease (chin crease)
