A large clinical trial is performed to evaluate the treatment response to statins in statin-naive patients admitted for angina chest pain. Patients are randomly assigned to moderate- or high-dose statin therapy upon discharge. At 3 months, the adverse cardiovascular event rate in the high-dose group compared to the moderate-dose group was 0.98 with a p value of 0.80. At 1 year, the relative risk ratio was 0.67 with a p value of 0.01. Treatment and follow-up compliance was high in both groups throughout the study interval. The difference between the 2 risk estimates is best explained by which of the following?

A. Confounding bias
B. Latency period
C. Observer bias
D. Selection bias
E. Selective survival bias

Test your knowledge, answer this
A 12-year-old girl is brought to the clinic by her parents for evaluation of decreased exercise tolerance and increased lethargy. Her parents say she becomes short of breath with minimal exertion, and for the past 2 weeks her gums have bled when she brushes her teeth. Temperature is 36. 7 C (98 F); blood pressure and pulse are normal. Physical examination demonstrates pallor and hepatosplenomegaly. An extensive workup indicates that the patient has acute lymphoblastic leukemia, and the parents are informed that she will need chemotherapy. The parents do not consent to the treatment plan because they believe that the side effects will be too severe and that their child's quality of life will be poor. Despite an ethics consultation and a lengthy discussion about the benefits of treatment and likely consequences if therapy is withheld, the parents insist on taking her home without scheduling follow-up treatment. Which of the following is the most appropriate next step in the management of this patient?
A. Comply with the parents' wishes
B. Obtain a court order for chemotherapy
C. Proceed with chemotherapy immediately
D. Provide supportive treatment only
E. Recommend transfer of care to another provider

Today’s MedBeats: Hospice-eligible patients frequently have renally cleared medications prescribed and at doses too high for their renal function. Analgesics, over-the-counter antihistamines, anticoagulants, anticholinergics have potential for significant adverse effects and higher vigilance is needed.

Today’s MedBeats: In the IMPACT-1 randomized clinical trial including adults with severe PTSD, intermittent treatment with TSND-201 (4 doses, 1 week apart) was generally well tolerated, safe, and associated with statistically significant and clinically meaningful improvements in PTSD symptoms that persisted well beyond the initial dosing period. Results support the further development of TSND-201 as a potential acute, intermittent treatment for PTSD. Together, these results demonstrate that TSND-201 has the potential to be a highly efficacious, rapid-acting and durable medication for PTSD. These promising results will require replication in larger upcoming studies.

Today’s MedBeats: Complication rates were low after vaginal hysterectomy and laparoscopic hysterectomy. The study results suggest that vaginal hysterectomy is not clearly a preferred route for hysterectomy over laparoscopic hysterectomy. The data support further investigation of the optimal surgical approach for hysterectomy with large, prospective clinical studies.

🩺 MedBeats: Voriconazole was not superior to itraconazole for treating chronic pulmonary aspergillosis and was associated with a significantly higher incidence of adverse events. Our findings support the continued use of itraconazole as the preferred therapy for chronic pulmonary aspergillosis, although voriconazole remains a reasonable alternative. The choice between the two agents should be guided by factors such as patient tolerance, drug availability, and cost considerations.
