If your child is playing a sport or managing a sport in school they will need a PHYSICAL FORM FILLED OUT. Please fill this form out before their wellness visit so the Dr can fill it out at that appointment. IF you drop it off after their wellness visit IT WILL TAKE 5-7 business days. (Everything highlighted must be filled out by parent or patient) Thank you!!! Also, just a reminder forms are $10.

Good morning,
If your child's school is requiring the UNIVERSAL HEALTH FORM here it is. Please fill out everything that is highlighted before your child's wellness visit (which they have once a year) so it can be filled out at that appointment.
