ESSENTIAL FUNCTIONS

Assess vital signs or other similar procedures per patient needs, at proper times and in accordance with physician orders.Document in the record and report unusual findings to Nurse promptly.

Communicate and assist residents with activities of daily living (ADL’s)(such as feeding, bathing, toileting, mobility, dressing) and scheduled activities. In general, residents will appear neat, clean and well-kept at all times. Support and encourage residents to maintain their independence. Report to nurse if knowledge of mental or physical conditions creates problems in self-care or completion of ADL’s in accordance to the care plan.

If a resident needs to be transferred, do so in manner as stated in care plan.

Responsible to know and practice the safety polices of the County. Perform all job tasks in a safe and responsible manner.

Consistently performs required treatments as assigned and documents correctly. Report unusual findings to nurse (i.e. splints, braces, cold compresses, etc.).

Participate with professional personnel in planning and carrying out patient/resident treatment plans and care plans both physical and psychiatric.

Conduct therapeutic and meaningful contact with residents. When required, perform patient charting and follow the treatment plan or care plans developed for individual residents.

Facilitate individual 1:1 patient interaction.

Work with the team to promptly and effectively respond to acting out, assaultive and self-abusive persons per facility policy and federal and state requirements. Recognize escalating patient/resident behaviors and intervene in a therapeutic manner using the least restrictive measures for patients who are a threat to themselves or others which include non-violent physical crisis prevention intervention techniques, de-escalation techniques and assisting the charge RN for patients that require restraint or seclusion.

Immediately report any and all incidents to the supervisor on duty. Ensure a safe and sanitary environment.

Complete an inventory of personal effects and items that an individual can harm themselves with, and secure upon admission. Complete body checks accurately and in a timely manner.

Conduct accurate supervision of those patients in need of frequent monitoring, such as: five (5) minute checks, su***de observation, elopement precautions or 1:1’s. Recognize unsafe situations and intervene and report them to their supervisor as appropriate.

Role models appropriate hygiene, grooming, social interactions and coping skills. Treats patient with respect and dignity.

Provide immediate first aid measures as required.

Transport and supervise patients/residents for appointments and out-of-center activities as required. Patients must be under direct observation of staff when not in the facility.

Additional essential duties require the MHT to support the patient’s mental health or rehabilitative recovery process while the patient is hospitalized and assist them in formulating a safety plan at discharge in collaboration with the nurse. In addition, the MHT will participate in active listening, worksheet exercises in problem solving and goal setting as assigned, and other emotional supportive care required during the patient’s hospitalization.

Supervise dining room during meal times.

May perform housekeeping procedures: make beds, put away clothing, empty wastebaskets, clean floors, bathrooms and work areas.

Participate in group activities and offer patient/resident activities within a therapeutic program.

Follow policies and procedures set forth at the facility.

OTHER JOB DUTIES

Meet on-going requirements to maintain current CNA certification. Participate in in-service and continuing education trainings as assigned. Provide documentation of all training activities.

Maintain cooperative working relationships and maintain appropriate professional boundaries with patients and residents.

Other duties as assigned.

EXPERIENCE, TRAINING, QUALIFICIATIONS
At a minimum a high school diploma. Minimum age 18 required. Must possess a valid Wisconsin Certified Nursing Assistant certification and maintain it per State requirements while employed by Wood County. Must be CPR certified.

Previous experience in dealing with mental health patients is preferred. Must be able to intervene and deal directly with highly agitated, depressed or assaultive patients in a non-retaliatory therapeutic manner.

Must be able to write legibly and communicate effectively, verbally. Demonstrated ability to maintain confidentiality.

Basic equipment used: EZ stand, EZ lift, gait belt, walker, wheelchair, sensor pad alarms, clip alarms, anti-rollback devices, seatbelts (alarmed or unalarmed), low beds, personal care items, mats for floor, scales, blood pressure cuff, thermometer, telephone, and stethoscope.

The purpose of this position is to evaluate and treat individuals in crisis and individuals with severe mental illness. Other duties include obtaining and implementing physician work orders and notifying appropriate parties of changes of condition. The work is performed under the supervision of the Head Nurse and Director of Nursing.

ESSENTIAL FUNCTIONS

1. Works with physicians, psychiatrists, psychologists and social workers as part of the treatment team.

2. Utilizes the nursing process for assessment, planning, implementing and evaluating both psychiatric and medical illnesses, including substance abuse.

3.Evaluates patients for response to treatments and medications. Monitors patient vital signs, observes patient for changes in behavioral or medical conditions and responds appropriately, responds promptly and effectively in a therapeutic manner, obtains and implements physicians orders and completes all assigned work in a timely manner.

4. Responds to resident/patient emergencies including psychiatric emergencies, critical illness, fire alarms, etc. Evaluates situations and takes appropriate action.

5.Prepares and implements patient/resident care plans and maintains complete and accurate patient/resident charts for treatments, medications, etc.

6. Administers oral medications, intramuscular and intravenous injections, range of motion exercises, catheterizations, enemas, other treatments and charts according to policies and procedures.

7. Provides consultation and referral information to other counties and facilities, psychologists, psychiatrists, law enforcement, community treatment teams, crisis intervention teams, hospital medical staff and prospective patients and families for potential admission of person to Norwood Health Center. Screens prospective admissions. Ensures admissions comply with procedure and required forms are complete.

8. Supervises work performance of subordinate nursing staff for proper technique, quality of care issues and use of equipment, materials and supplies. Reviews subordinates’ chart recordings for accuracy. Prepares written evaluations. Conducts staff in-service and new staff orientations. Occasionally needs to participate in disciplinary process of subordinate staff.

9. Converses with patients to assure they are comfortable. Explains recovery progress, therapies, treatments, medications and changes in medication. Answers resident’s request for assistance.

10. Communicates and interacts professionally, effectively and tactfully with the residents, visitors, families, peers, referral sources, community staff, crisis and supervisors.

11. Answers questions and complaints from family members. Notifies families regarding patient needs for personal items such as clothing, glasses, etc.

12. Participates in facility committees regarding nursing care as assigned. Attends and participates in staff meetings and in-services. Attends and participates in continuing educational seminars, conferences, workshops, etc.

13. Orders medications, supplies and materials.

B. OTHER JOB DUTIES

1. Assists residents in eating, toileting, ambulating, repositioning and other daily living tasks. Arranges transportation as needed. Tidies patient rooms.

2. Assists in subduing and/or restraining agitated and/or violent patients, as needed.
3. Distributes meals and snacks.

4. Keeps work environment clean and organized and answers telephone.

5. Staffs the unit, maintaining safety and fiscal responsibility.

6. Acts as administrative charge for entire facility in the absence of Director of Nursing, Administrator or Client Services Manager.

Job Qualifications:

Associate degree in Nursing, one to two years nursing experience, preferably in behavioral health, or any combination of education and experience that provides equivalent knowledge, skills and abilities. Wisconsin Registered Nurse license required.
