The West Virginia Department of Health and Human Resources, Bureau for Behavioral Health and Bureau for Medical Services are seeking input to gain understanding of the West Virginia Intellectual/Developmental Disability (I/DD) system and supports for people with I/DD who have complex mental health and behavioral needs.

We are looking for family members, advocates and providers who work directly with people with I/DD to take part in this survey. Responses are anonymous. If you would like a hard copy of the survey sent to you free of charge, please email Lisa Dillon at ldillon@trinityservice.org.
