Patient Visit & Insurance Guidelines

To help ensure you receive the appropriate care and avoid unexpected charges, please review the following information about visit types and insurance coverage:

🩺 Commercial Insurance Patients
Most commercial insurance plans cover one annual physical exam per year, which may include routine screening labs. This visit is focused on preventive care and overall wellness.
• Concerns such as new symptoms, chronic condition management, or specific medical problems should be addressed during a separate problem-focused visit.
• Addressing problems during a physical may result in additional charges depending on your insurance plan.

🩺 Medicare Patients
Medicare covers an Annual Wellness Visit (AWV) once per year. This visit is commonly done at a regularly scheduled office visit with our wellness nurse.
• This visit focuses on preventive care, health risk assessment, and care planning.
• It is not a traditional physical exam.
• Labs must be ordered with an appropriate diagnosis to meet Medicare coverage requirements.

🩺 Medicare Advantage Patients
Depending on your specific plan, you may be eligible for:

✔️ An Annual Wellness Visit (AWV)
✔️ A Preventive Physical Exam and both can be performed at the same visit. In addition a problem focused visit can be completed and all 3 visits can be billed the same day.

Coverage varies by plan, so please check with your insurance provider if you have questions.

The office will be open both Friday and Monday. The office will be closed Saturday.

Staffing is split so that everyone gets a long holiday weekend but allows the office to not be closed. So your favorite provider or nurse may not be there one of those days.

At Martinsville Family Medicine, we are proud to offer Medicare’s Chronic Care Management (CCM) Program to help patients living with two or more chronic conditions receive coordinated, continuous care — even between office visits.

💙 What is CCM?
CCM is a Medicare-supported program designed to give you extra support outside of your regular appointments so you can stay healthier and avoid complications.

As a participant in our CCM program, you’ll receive:

• A personalized care plan tailored to your health needs and goals
• Ongoing support from our care team between visits
• Medication management with consultation from a clinical pharmacist to ensure your prescriptions are safe, effective, and working well together
• Regular check-ins to answer questions and monitor progress
• Assistance connecting with helpful community resources
• Support aimed at preventing hospital visits and complications
• Guidance and encouragement to stay actively involved in your care

⏳ The average duration of the program is about 4 months, but it can be adjusted to help you reach your specific health goals.

💳 Because this program is offered through Medicare, patients are responsible for a 20% co-pay if no secondary insurance coverage is available.

📊 The Virginia Department of Health collects de-identified data from participants to help evaluate how programs like CCM improve care in rural communities.

Medicare and medicare advantage plans request all patients to undergo an Annual Medicare wellness exam. We follow guidelines set both by Medicare/insurance companies and practice best clinical practice based on guidelines.

Medicare advantage plans would like us to see their patients at least 1-2 times a year. Seeing patients routinely reduces overall medical costs by allowing for early detection and prevention of diseases.

The clock drawing test is a portion of a Medicare annual wellness.

Prior to turning 65 and going on Medicare you should be getting an annual physical. Once on Medicare that yearly exam turns into an annual wellness exam.

The office will open at 8 am Monday and operate on normal hours.

The parking lot has been scraped but there is still some residual snow/ice so use caution.

Thank you to all the staff for working during these weather events and particularly Freddie Daniels for all you do to keep the office open and running smoothly.

The office will be open normal hours on Tuesday.

Some staff may be late to arrive so be patient with the office.

The parking lot was scraped and the area closest to the building is fairly clear but down the parking lot is still covered.

It will be very cold in the morning so there will be icy spots.

Use caution if you decide to venture out.

The office will have both in person and virtual visits available throughout the day.
