We are hiring a compassionate RN Case Manager to join our Case Management team, to support pediatric patients receiving Private Duty Nursing (PDN) services in the home.

What You’ll Do:
Coordinate care for pediatric PDN patients
Perform in-home assessments and supervisory visits.
Develop and manage individualized care plans.
Support and guide bedside nurses.
Partner with families and physicians to ensure quality & continuity of care.

Qualifications:
Active Texas RN license
1+ year nursing experience (PDN preferred)
Strong clinical, communication and writting skills.
Excellent time management skills.

Schedule:
Full-Time: Monday–Friday (Some on-call)

Little Warriors Pediatric Home Care is seeking a compassionate and reliable Private Duty Nurse (PDN) with hands-on experience in seizure precautions to provide one-on-one care for a pediatric patient in a home setting.

Responsibilities:
Monitor and manage seizure activity
Implement and maintain seizure precautions
Administer medications as prescribed
Provide airway support and emergency intervention if needed
Document care and communicate with family and healthcare team
Assist with daily living activities as required
Requirements:
Active LVN license in good standing
Experience with seizure disorders and emergency response.
Current CPR certification.
Strong assessment and communication skills.
Home health or pediatric private duty experience preferred.
