Physician Assistant (PA) / Nurse Practitioner (NP)
Internal Medicine / Adult & Geriatric Medicine – Outpatient Private Practice

Position Summary
Our well-established private Internal Medicine and Adult/Geriatric Medicine practice is seeking a skilled and compassionate Physician Assistant or Nurse Practitioner to join our team.
This position is outpatient only, with no hospital rounding or after-hours call.(some home call?) The ideal candidate will provide comprehensive primary and preventive care to adult and older adult patients, focusing on chronic disease management, wellness, and healthy aging in a collaborative and patient-centered environment.

Key Responsibilities
• Perform comprehensive patient evaluations, including histories, physical exams, and medication reconciliations.
• Manage chronic medical conditions such as hypertension, diabetes, COPD, heart failure, and hyperlipidemia.
• Provide acute care visits for common conditions (e.g., respiratory infections, musculoskeletal complaints, skin issues).
• Order and interpret laboratory tests, imaging, and other diagnostic studies.
• Develop, implement, and monitor personalized treatment plans under the supervision of or in collaboration with the attending physician.
• Deliver preventive care and patient education, including screenings, vaccinations, and lifestyle counseling.
• Perform office-based procedures as appropriate (e.g., EKG interpretation, joint injections, wound care).
• Maintain accurate and timely documentation in the electronic health record (EHR).
• Participate in team meetings, quality improvement initiatives, and care coordination with specialists and community services.
• Foster long-term relationships with patients, focusing on continuity of care and excellent patient experience.

Qualifications
Required:
• Current, unrestricted state license as a Physician Assistant or Nurse Practitioner.
• Certification by the NCCPA (PA) or AANP/ANCC (NP).
• Current DEA license (or eligibility to obtain).
• BLS certification.
• Minimum 1 year of primary care or internal medicine experience (exceptional new graduates will be considered).
Preferred:
• Experience managing older adult patients and complex chronic diseases.
• Familiarity with value-based or team-based care models.
• Strong communication and interpersonal skills.
• Comfort working both independently and collaboratively in a small-practice environment.

Work Schedule
• Outpatient only – no hospital or nursing home responsibilities.
• Full-time (Monday–Friday, business hours).
