Heres ankle level wound , at look it wont show much , carful eye exam shows the black discoloration, and on debridement intraoperatively you can see , infected torn tendons , deeper wound . Its sometimes essential to do debridement on static healing wounds and delayed healing wounds.
It was so worth it.

Diabetic infection was admitted and amputation done 4 months ago by genetal surgery. Patient was discharged with open wound. No follow up. Presented 4 months later with chronic open wound and bone infection almost of all his forefoot bones.

No mri was done initially
No vascular studies
No proper antibiotics
