Subtle Lisfranc injuries represent a spectrum of midfoot instability, and conservative or percutaneous treatment risks missing unstable joints. Open reduction with careful inspection and fixation of all unstable joints is often necessary to prevent chronic pain, arthritis, and deformity.
CT scans revealed fractures in over half (56%) of cases that were not visible on plain X-rays, showing that these injuries are frequently underdiagnosed. Importantly, isolated Lisfranc ligament rupture alone did not occur—additional ligaments or joints were always involved.
