Subtle Lisfranc injuries represent a spectrum of midfoot instability, and conservative or percutaneous treatment risks missing unstable joints. Open reduction with careful inspection and fixation of all unstable joints is often necessary to prevent chronic pain, arthritis, and deformity.

CT scans revealed fractures in over half (56%) of cases that were not visible on plain X-rays, showing that these injuries are frequently underdiagnosed. Importantly, isolated Lisfranc ligament rupture alone did not occur—additional ligaments or joints were always involved.

You can be flat footed from previous calc fracture healed wrong or from mid foot collapsing. Each one has different mechanisms and results on your presentation but eventually arthritis and pain.

Presented with infected cubiod with open wound and sever adductovarus foot. Staged procedures. Was on ex fix. Now finally walking with im nail. Limb salvage. Oh I missed one abx bead in mid foot. Might take it out.

65 y o f. Charcot Ankle fracture in Mexico. Fixed by big k wires, then another provider ( ortho) placed new k wires . It was painful with open wound. Removed k wires and placed im nail and fused Ankle and stj. Patient back to walking w no pain. K wires was impeded next to the tarsal tunnel.

When you have achellis tendon pain , contracture this s how your body will compensate for it. I loved it how this almost always true. To relieve pain you will walk on your tip toes or foefoot. Thats how a secondary spot of pain starts in your forefoot, or balls of the foot.

A very painful tendinitis at the insertion of the achellis, issue in this patient, we rather go with conservative measures or with very minimal invasive surgery if have to. Swelling due to Venous stasis can be a major healing obstacle.
