At Highland Oaks Health Center, every rehabilitation journey is guided by individualized care and measurable outcomes. Daisy Irvin was admitted in December and received Physical, Occupational, and Speech Therapy tailored to her specific needs.

👏 Over the course of 23 days, she made significant progress, improving her functional outcome score from admission to discharge and successfully transitioning to the next level of care.

Stories like Daisy’s reflect the dedication of our interdisciplinary rehab team and their commitment to helping residents regain independence and confidence every step of the way. 💙
