The Centers for Medicare & Medicaid Services (CMS) has finalized a rule that closes a financing loophole previously used by some states to generate higher federal Medicaid matching funds by taxing Medicaid Managed Care Organizations (MCOs) at higher rates than commercial health plans.

With the rule taking effect on April 3, 2026, seven states will need to restructure their tax models, and the federal government estimates this change could save nearly $78 billion over the next decade.
