Dear Patients,
It is with mixed emotions that I am announcing that Tyson's Corner Pediatrics, located at 8365 Greensboro Drive, Suite A, McLean, Virginia 22102, will close effective November 10, 2022. The time that I have spent with you has been personally rewarding and a joy, due to the many wonderful patients which I had the privilege of caring for.
While we are available to provide you with medical Care until November 10, 2022, you will need to secure a new physician prior to that date to ensure continuous care. As a member of Privia Medical Group, there are available Pediatric Practices available to provide you care with the highest quality you've come to expect.
All medical records are confidential and protected by federal privacy and security regulations. With your authorization, a copy of your medical record(s) can be released to you or transferred to a new physician. If you transfer to another Privia Medical Group Provider, your records will automatically be available to your new provider once you have registered with the new Practice.
If you wish to receive a copy of your medical records and/or request that they be transferred to another provider, please complete the authorization to release medical records form and return it to:
medicalrecords@priviahealth.com or fax to 202-403-3099.
This would be for records from December 13, 2013 to November 9, 2022.

You may be charged a reasonable fee to cover the cost of copying and/or transferring your medical records.
